摘要
目的分析乌司他丁联合利奈唑胺治疗重症肺炎的临床效果。方法抽取开封市人民医院2018年2月至2021年2月收治的重症肺炎患者60例,按随机数字表法分为联合组和单一组,每组30例。单一组采用利奈唑胺治疗,联合组采用乌司他丁联合利奈唑胺治疗。比较两组的疗效及治疗前后血气水平、肺功能、临床肺部感染评分(CPIS)、急性生理和慢性健康评分Ⅱ(APACHEⅡ)评分、应激激素、炎性因子水平。结果联合组总有效率(93.33%,28/30)高于单一组(73.33%,22/30),P<0.05。治疗后,联合组氧合指数、动脉血氧分压、最高呼气流速、每分钟最大通气量均高于单一组(P均<0.05);CPIS、APACHEⅡ评分低于单一组(P<0.05);去甲肾上腺素、血管紧张素Ⅱ、高迁移率族蛋白B1、血清淀粉样蛋白A表达水平均低于单一组(P均<0.05)。结论乌司他丁联合利奈唑胺治疗重症肺炎的效果确切,可恢复血气水平及肺功能,减轻炎性刺激和肺部感染程度,缓解病情。
Objective To analyze the clinical effect of ulinastatin combined with linezolid in the treatment of severe pneumonia.Methods A total of 60 severe pneumonia patients treated in Kaifeng People’s Hospital from February 2018 to February 2021 were selected and divided into combination group and single group by random number table method,with 30 cases in each group.The single group was treated with linezolid,and the combination group was treated with ulinastatin combined with linezolid.Efficacy,blood gas level,lung function,clinical pulmonary infection score(CPIS),acute physiology and chronic health evaluationⅡ(APACHEⅡ)score,stress hormones and inflammatory factors were compared between the two groups.Results The total effective rate of the combination group(93.33%,28/30)was higher than that of the single group(73.33%,22/30),P<0.05.After treatment,the oxygenation index,arterial oxygen partial pressure,peak expiratory flow and maximal voluntary ventilation per minute in the combination group were higher than those in the single group(all P<0.05).After treatment,CPIS and APACHEⅡscores in the combination group were lower than those in the single group(P<0.05).After treatment,the expression levels of norepinephrine,angiotensinⅡ,high mobility group protein B1 and serum amyloid A in the combination group were lower than those in the single group A(all P<0.05).Conclusions The addition of ulinastatin combined with linezolid in the treatment of severe pneumonia is effective,which can restore blood gas level and lung function,reduce inflammatory stimulation and pulmonary infection,and alleviate the disease.
