摘要
目的探讨混合现实技术在骶骨骨折经皮骶髂关节螺钉固定术教学中的应用效果。方法收集1例采用经皮骶髂关节螺钉固定术治疗的DenisⅡ型单纯骶骨骨折患者的影像学资料(腰椎X线片、腰椎CT+三维重建),建立三维解剖模型及手术演示模型。将重点手术步骤、主要操作方法等信息导入HoloLens平台。选取24名在内蒙古医科大学附属医院骨科进行课间实习轮转、无骨科操作经历的医学生,依据不同授课方式随机分为混合现实组和传统教学组,每组12人,分别采用混合现实辅助教学和传统教学方式进行授课。对学生的学习主动性、三维立体感、教学满意度、学习专注性及知识掌握能力进行问卷调查和课后测试。结果在学生学习主动性、三维立体感、教学满意度评分方面,混合现实组高于传统教学组(P<0.05);而在学习专注性、知识掌握能力评分方面,两组比较,差异无统计学意义(P>0.05)。混合现实组和传统教学组课后测验成绩分别为(88.2±4.4)、(78.2±5.7)分,两组比较,差异有统计学意义(t=4.811,P<0.001)。结论将混合现实技术应用于骶骨骨折固定术教学中,医学生能更清晰地理解手术部位解剖结构,更加真实地体验手术操作过程,提高主动学习效率和兴趣,增强动手能力,提升骨科临床教学效果。
Objective To explore the application of mixed reality technique in clinical teaching of percutaneous screw fixation of sacroiliac joint for sacral fracture.Methods Imaging data including lumbar plain film,CT and 3D reconstruction of 1 patient of simple sacral fracture with type Denis II treated by percutaneous screw fixation of sacroiliac joint were collected to establish 3D anatomical model and operation demonstration model.Important surgical procedures as well as major operation methods were imported into the HoloLens platform.Without any orthopaedic practice experience,a total of 24 medical students who underwent clinical practice rotation in orthopaedic department of the Affiliated Hospital of Inner Mongolia Medical University were randomly divided into two groups,mixed reality group and traditional teaching group(n=12).Students in mixed reality group were taught by mixed reality technology assisted teaching,while those in traditional teaching group were taught by traditional teaching methods.Questionnaire survey was conducted on five aspects including learning initiative after class,three-dimensional sense,teaching satisfaction,learning concentration as well as knowledge mastery ability,and then the test was carried out after the class.Results Scores of learning initiative after class,three-dimensional sense and teaching satisfaction of students in mixed reality group were all higher than those in traditional teaching group(P<0.05),while there were no statistical differences in scores of learning concentration and knowledge mastery ability of students between the two groups(P>0.05).The scores in after-class test were(88.2±4.4)and(78.2±5.7)points in mixed reality group and traditional teaching group respectively,the difference between two groups was statistically significant(t=4.811,P<0.001).Conclusion The application of mixed reality technology in teaching of sacral fracture fixation enables students to have a clearer understanding of the anatomical structure and a more real experience of the operation pro
