摘要
目的分析血清癌胚抗原125(CA125)、可溶性间皮素相关蛋白(SMRP)、B7-H4与经阴道彩色多普勒超声(TV-CDFI)联合检测用于卵巢癌诊断的敏感度与特异性。方法本研究为前瞻性研究,以我院2019年6月~2021年6月进行诊断并进行治疗的126例卵巢癌患者为研究对象,另选取126例同期健康体检患者为对照组,比较观察组及对照组不同分期患者的血清CA125、SMRP、B7-H4与TV-CDFI指标的差异,分析血清CA125、SMRP、B7-H4与TV-CDFI联合检测对卵巢癌诊断的效能。结果观察组患者的RI(t=59.894,P<0.001)、PI(t=31.512,P<0.001)低于对照组,PSV(t=43.806,P<0.001)、EDV(t=13.692,P<0.001)高于对照组,观察组患者的CA125(t=79.903,P<0.001)、SMRP(t=26.878,P<0.001)、B7-H4(t=57.275,P<0.001)高于对照组;Ⅰ~Ⅱ期患者RI(t=51.439,P<0.001)、PI(t=7.631,P<0.001)高于Ⅲ~Ⅳ期,PSV(t=22.288,P<0.001)、EDV(t=4.423,P<0.001)低于Ⅲ~Ⅳ期;Ⅰ~Ⅱ期患者CA125(t=7.302,P<0.001)、SMRP(t=2.034,P=0.044)、B7-H4(t=7.217,P<0.001)低于Ⅲ~Ⅳ期;通过联合检测分析,TV-CDFI联合血清CA125、SMRP、B7-H4对卵巢癌诊断的特异性高于单独检测;ROC曲线分析显示,TV-CDFI联合血清CA125、SMRP、B7-H4对卵巢癌诊断的曲线下面积高于单独检测。结论血清CA125、SMRP、B7-H4与TV-CDFI联合检测对于卵巢癌诊断的特异性较高。
Objective To analyze the sensitivity and specificity of combined serum CA125,SMRP,B7-H4 and TV-CDFI in the diagnosis of ovarian cancer.Methods A prospective study was conducted in this paper,in which 126 patients with ovarian cancer were diagnosed and treated in our hospital from June 2019 to June 2021 were selected as the research group,and another 126 patients with health physical examination were selected as the control group to compare the differences of serum CA125,SMRP,B7-H4 and TV-CDFI indexes between the observation group and the control group with different stages,and to analyze the differences of serum.The efficacy of combined detection of serum CA125,SMRP,B7-H4 and TV-CDFI in the diagnosis of ovarian cancer was analyzed.Results Patients in the observation group had lower RI(t=59.894,P<0.001)and PI(t=31.512,P<0.001)and higher PSV(t=43.806,P<0.001)and EDV(t=13.692,P<0.001)than the control group,and patients in the observation group had higher CA125(t=79.903,P<0.001),SMRP(t=26.878,P<0.001),and B7-H4(t=57.275,P<0.001)were higher than those in the control group;The RI(t=51.439,P<0.001)and PI(t=7.631,P<0.001)in patients at stageⅠ-Ⅱwere significantly higher than those at stageⅢ-Ⅳ.The PSV(t=22.288,P<0.001)and EDV(t=4.423,P<0.001)in patients at stageⅠ-Ⅱwere significantly lower than those at stageⅢ-Ⅳ.The CA125(t=7.302,P<0.001),SMRP(t=2.034,P=0.044)and B7-H4(t=7.217,P<0.001)in patients at stageⅠ-Ⅱwere significantly lower than those at stageⅢ-Ⅳ.By combined assay analysis,the specificity of TV-CDFI combined with serum CA125,SMRP and B7-H4 in the diagnosis of ovarian cancer was significantly higher than that of individual detection.Through ROC curve analysis,the area under the curve of TV-CDFI combined with serum CA125,SMRP and B7-H4 in the diagnosis of ovarian cancer was significantly higher than that of individual detection.Conclusion The specificity of the combination of serum CA125,SMRP,B7-H4 and TV-CDFI was higher for the diagnosis of ovarian cancer.
