针灸对腰神经根受压模型大鼠疼痛症状的改善及对腰受损神经根组织的炎症抑制作用 被引量:3

摘要 目的:研究针灸对腰神经根受压模型大鼠疼痛症状的改善及对腰受损神经根组织的炎症抑制作用。方法:取50只SD大鼠,随机选取10只仅做假手术,设为假手术组,其余40只建立腰神经根受压模型,将建模成功的31只大鼠随机分为模型组(10只)、针灸组(10只)和阳性药物组(11只)。建模后第4天针灸组针刺L_(5)受压神经根右侧夹脊穴,假手术组与模型组每天仅固定20 min,不实施针灸,阳性药物组采用布洛芬缓释胶囊60 mg/(kg·d)灌胃。测定大鼠热痛阈值;测定腰受损神经根组织白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)浓度;测定腰受损神经根组织Toll样受体4(TLR4)、髓样细胞分化因子88(MyD88)、核转录因子κB p65(NF-κB p65)mRNA及蛋白表达量。结果:与模型组比较,针灸组、阳性药物组热痛阈值均升高,差异具有统计学意义(P<0.05);与针灸组比较,阳性药物组热痛阈值均升高,差异具有统计学意义(P<0.05);与模型组比较,针灸组、阳性药物组腰受损神经根组织IL-6、TNF-α浓度、TLR4、MyD88、NK-κB p65 mRNA及蛋白表达量均降低,差异具有统计学意义(P<0.05);与针灸组比较,阳性药物组腰受损神经根组织IL-6、TNF-α浓度、TLR4、MyD88、NK-κB p65 mRNA及蛋白表达量均降低,差异具有统计学意义(P<0.05)。结论:针灸可有效减轻腰神经根受压模型大鼠疼痛,抑制炎症反应,改善腰受损神经根病理变化,推测其作用机制与抑制TLR4/MyD88信号通路有关。 Objective:To study the effect of acupuncture on pain improvement and inflammation inhibition in model rats of lumbar nerve root compression.Methods:10 SD rats were assigned into the sham operation group;40 SD rats were established the model of lumbar nerve root compression,among which 31 SD rats with successful modeling were randomly divided into the model group(n=10),the acupuncture group(n=10)and the positive drug group(n=11).On the 4th day after modeling,the acupuncture group was treated with needling Jiaji points on the right side of the L5 compressed nerve root,the sham operation group and the model group were only fixed for 20 minutes without acupuncture,and the positive drug group was treated with Ibuprofen Sustained-Release Capsules 60 mg/(kg·d).The heat pain threshold of rats were determined,the concentrations of IL-6 and TNF-αof lumbar nerve root tissues were measured,and the mRNA and protein expressions of Toll-like receptor 4(TLR4),myeloid cell differentiation factor 88(MyD88)and nuclear transcription factorκB p65(NF-κB p65)in lumbar nerve root tissues were detected.Results:Compared to that in the model group,the heat pain threshold was increased in the acupuncture group and in the positive drug group(P<0.05);the heat pain threshold was higher in the positive drug group than that in the acupuncture group(P<0.05).The concentrations of IL-6 and TNF-α,as well as the mRNA and protein expressions of TLR4,MyD88 and NK-κB p65 were all significantly decreased in the acupuncture group and in the positive drug group compared to those in the model group(P<0.05);which were lower in the positive drug group than those in the acupuncture group(P<0.05).Conclusion:Acupuncture can effectively reduce the pain,inhibit the inflammatory response and improve the pathological changes of the injured lumbar nerve root in model rats of lumbar nerve root compression.It is speculated that its mechanism is related to the inhibition of TLR4/MyD88 signaling pathway.
