摘要
目的:使用频次统计法、复杂网络分析法探讨现代临床中针刺治疗脑卒中后认知障碍的选穴规律。方法:收集2020年3月31日前在CBM、维普Vip、CNKI、Wanfang Data、Embase、the Cochrance Library、SCI Expanded、PubMed及OVID中发表的相关临床研究文献,采用Microsoft Excel 2010建立数据库,进行数据的频次统计,运用Gephi 0.9.2软件建立复杂网络模型并分析其拓扑结构。结果:共纳入55个腧穴处方。针灸治疗PSCI的整体选穴多属督脉、胆经,特定穴归属则是以交会穴、五输穴为主。构建的腧穴处方复杂网络具有小世界效应和无标度特性,重要腧穴以百会、神庭、内关、四神聪、风池、足三里及三阴交为主,腧穴配伍也以此7穴最为常见,配穴方法则以局部配穴和上下配穴为主;运用k-core优化腧穴处方,16个核心腧穴以局部头部督脉腧穴为主,配合心肝肾脏腑原穴、脾胃经补益之穴,再辅以各经交会穴总络全身,且87.5%为特定穴,以交会穴居多。结论:局部选穴是现代临床中针刺治疗脑卒中后认知障碍的首选,局部配穴和上下配穴最为常用,特定穴是其核心用穴。
Objective:To explore the rules of acupoints selection in the modern acupuncture and moxibustion treatment of post-stroke cognitive impairment(PSCI)by using frequency statistics and complex network analysis.Methods:Relevant clinical research literature was collected from the databases of CBM,Vip,CNKI,Wanfang Data,Embase,the Cochrance Library,SCI Expanded,PubMed and OVID before March 31,2020.A database was established by Microsoft Excel 2010 to carry out Data frequency statistics.Gephi 0.9.2 was used to build a complex network model and analyze its topology.Results:A total of 55 acupoint prescriptions were included.The overall selection of acupoints in the treatment of PSCI mostly belonged to Du meridian and Gallbladder meridian,whereas the specific points were mainly crossing points and five-Shu points.The complex network of acupoints prescriptions has small-world and scale-free characteristics.The important acupoints were Baihui(DU20),Shenting(DU24),Neiguan(PC6),Sishencong(EX-HN1),Fengchi(GB20),Zusanli(ST36)and Sanyinjiao(SP6).The most common acupoints compatibility was included these seven points,and local acupoints match and upper-lower acupoints match were the dominate ones;k-core was used to optimize acupoints prescriptions,the 16 core acupoints were mainly located in Du meridian of the head,with the Yuan-source points of Heart,Liver and Kidney meridians,as well as the tonifying points of Spleen and Stomach meridians,and supplemented by the crossing points of the meridians.87.5%of them were specific acupoints,of which the crossing points were the most common.Conclusion:Local acupoints election is the first choice of acupuncture and moxibustion treatment for PSCI in modern clinical practice,local points match and upper-lower points match are most commonly used,and specific points are the core points.
