摘要
目的了解我国日间手术开展的基本情况并思考其规范化发展的关键要素。方法对2019年国家医疗质量管理与控制信息网收集的日间手术相关指标数据依照服务能力、质量与安全、效率和效益3大维度对日间手术的开展情况进行描述性分析。结果三级医疗机构是日间手术开展的主力军,其三、四级日间手术占比高于二级医疗机构;但在所统计的主要质量与安全指标中,二、三级医疗机构各有千秋;二、三级医疗机构日间手术诊断常见病种相似且同一病种的次均费用差异不明显。结论日间手术的规范化发展关键是明确1个定义、建立2套体系,即在明晰、统一的日间手术概念的基础上,建立系统、科学的管理和监督评价体系。配套的医保政策的护航必不可少,院内绩效制度的支持和延续性医疗服务能力也必不可少。
Objective To describe the status quo of day surgery implementation on the basis of which key factors to facilitate its normative implementation were addressed.Method Descriptive analysis was conducted in three main dimensions,service capability,quality and safety,efficiency and effectiveness,based on day surgery data collected by National Clinical Improvement System(NCIS)in 2019.Result The majority of day surgery occurred in tertiary medical institutions which also displayed higher proportion of LevelⅢand LevelⅣday surgery than secondary hospitals.As for the quality and safety indicators,secondary and tertiary medical institutions all demonstrated their advantages and shortcomings.While similarities were found in commonly diagnosed types of day surgery diseases in secondary and tertiary medical institutions and their average cost of a given disease showed no significant difference.Conclusion The key to promoting normative development of day surgery lies in a clear definition and two sets of management systems,which indicate to establishing a set of systematic management systems as well as a set of service supervision and evaluation system on the ground of a unified definition of day surgery.Additionally,supportive medical insurance policy,rewarding hospital performance appraisal system and capability of continuing medical service all play a crucial role.
