摘要
目的探究肺炎克雷伯菌铁载体含量与耐碳青霉烯类抗生素的关系。方法采用铬天青S(CAS)检测法确定肺炎克雷伯菌是否产铁,紫外分光光度法检测铁载体的相对含量,根据中位数将60株临床分离菌分为高产铁载体组(30株)和低产铁载体组(30株);用聚合酶链式反应(PCR)检测铁载体基因和碳青霉烯类耐药基因;采用改良碳青霉烯类失活法(mCIM)实验对碳青霉烯酶的型别进行初步分类。分析肺炎克雷伯菌碳青霉烯类耐药与其铁载体含量的关系。结果60株肺炎克雷伯菌均产生铁载体,低产铁载体组的铁载体含量低于高产铁载体组(t=9.530,P<0.01);PCR法共检出27株碳青霉烯耐药基因阳性菌,mCIM试验的敏感度与特异度分别为92.59%和90.90%。高产铁载体组耐药率低于低产铁载体组(χ^(2)=9.600,P<0.01)。根据药敏结果将60株受试菌分耐药组和敏感组各30株,碳青霉烯类药物敏感组菌株较耐药组菌株铁载体产量更高(t=2.481,P<0.05)。结论肺炎克雷伯菌铁载体的产量与碳青霉烯类抗生素耐药性有关。
Objective To study the association between the siderophore capacity of Klebsiella pneumoniae and resistance of carbapenem antibiotics.Methods Chromium azurol S(CAS)was used to determine whether Klebsiella pneumoniae produced iron,The relative content of siderophore produced was detected by ultraviolet spectrophotometry.According to the median of statistics,60 clinical isolates were divided into high siderophore produced group(30 strains)and low yield group(30 strains).Siderophore genes and carbapenem resistance genes were detected by polymerase chain reaction(PCR)method,and carbapenem types were classified by modified carbapenem inactivation method(mCIM)test.The relationship between carbapenem resistance and siderophore produced of Klebsiella pneumoniae was analyzed by SPSS statistical software.Results All 60 strains of Klebsiella pneumoniae produced siderophore,and the yield of siderophore in the low yield group was lower than that in the high production group(t=9.530,P<0.01).A total of 27 strains of carbapenem resistance gene positive bacteria were detected by PCR method,and the sensitivity and specificity of mCIM test were 92.59%and 90.90%,respectively.The drug resistance rate in the high siderophore produced group was significantly lower than that in the low yield group(χ^(2)=9.600,P<0.01).According to the results of drug sensitivity,60 strains of bacteria were divided into 30 resistant groups and 30 sensitive groups.The strains in the sensitive group had stronger siderophore production capacity than the carbapenem resistant group(t=2.481,P<0.05).Conclusion The siderophore produced yield of Klebsiella pneumoniae is related to carbapenem resistance.
