斑点追踪技术联合三维超声心动图评估冠心病患者PCI术后左心室心肌功能和同步性临床研究 被引量:5

摘要 目的:应用二维斑点追踪技术(Two-dimensional speckle tracking imaging,2D-STI)和三维超声心动图(Realtime three-dimensional echocardiography,RT-3DE)对前降支冠状动脉狭窄的冠心病患者PCI术后进行左心室心肌功能和同步性的临床研究。方法:选取2019年5月至2020年12月经冠状动脉造影检查确诊冠心病且行PCI治疗的50例患者为研究对象,分别于术前1周、术后1个月、术后6个月行超声心动图检查。采用2D-STI和RT-3DE技术测量患者3个不同时期的左心室舒张末期容积(LVEDV)、收缩末期容积(LVESV)、射血分数(LVEF)、纵向应变(GLS)、圆周应变(GCS)、16节段达最小收缩期容积时间的最大时间差(Tmsv-16-diff)和标准差(Tmsv-16-SD)以及校正后占心动周期的百分比(Tmsv-16-diff%、Tmsv-16-SD%)等同步性相关指标;分析术后6个月应变值(GLS、GCS)与LVEF的相关性。结果:患者术后1个月、术后6个月LVEDV、LVESV、Tmsv-16-SD等同步性指标均低于术前1周,患者术后1个月、术后6个月LVEF、GLS、GCS测值均高于术前1周,差异均有统计学意义(P<0.001);患者术后6个月2D-STI和RT-3DE的测值均较术后1个月明显改善,差异有统计学意义(P<0.001),GLS与LVEF呈负相关(r=-0.733,P<0.001),GCS与LVEF呈负相关(r=-0.334,P<0.05)。结论:2D-STI联合RT-3DE可以更好反映冠心病患者心肌功能变化及同步性的改变,为评价PCI术后疗效及患者预后提供有价值的信息。 Objective:To study the left ventricular myocardial function and synchronization of coronary heart disease patients with anterior descending coronary artery stenosis after PCI with.two dimensional speckle-tracking technique(2D-STI)and three dimensional echocardiography(RT-3DE).Methods:Fifty cases with severe anterior descending coronary artery stenosis demonstrated by coronary angiography and requiring PCI treatment were included as PCI group.Echocardiography was performed 1 week before surgery,1 month after surgery,and 6 months after surgery.The left ventricular end-diastolic volume(LVEDV),end-systolic volume(LVESV),ejection fraction(LVEF),longitudinal strain(GLS),circular strain(GCS)and the maximum time difference between 16 levels to the minimum systolic volume in PCI group were measured by 2D-STI and RT-3DE techniques 16-diff,standard deviation(Tmsv-16-SD),and adjusted percentage of cardiac cycle(Tmsv-16-diff%,Tmsv-16-SD%).The differences of parameters in different time periods were compared,and the correlation between strain(GLS,GCS)and LVEF 6 months after operation was analyzed.Results : The synchronization indexes of LVEDV,LVESV and Tmsv-16-SD in the patients 1 month and 6 months aftersurgery were significantly lower than those in the patients 1 week before surgery;and the measurements of LVEF,GLSand GCS in the patients 1 month and 6 months after surgery were significantly higher than those in the patients 1 weekbefore surgery,with statistical significance(P<0. 001). The measured values of patients at 6 months after operationwere significantly improved compared with those at 1 month after operation,with statistical significance(P<0. 001).There was a significant negative correlation between GLS and LVEF(r=-0. 733,P<0. 001),and there was a negativecorrelation between GCS and LVEF(r=-0. 334,P<0. 05). Conclusion : 2D-STI combined with RT-3DE can betterreflect the changes of myocardial function and synchronization in patients with coronary heart disease,and providevaluable information for evaluating the efficacy
