大连市社区中老年人群夜间睡眠时间与糖耐量减低发生风险的队列研究 被引量:4

Association between nighttime sleep duration and incidence of impaired glucose tolerance in middle-aged and elderly residents in Dalian:a cohort study

摘要 目的探讨大连市社区中老年人群夜间睡眠时间与IGT发生风险的关系。方法选取2011年7~12月≥40岁大连市社区常住居民6358名开展基线调查,其中2645名完成3年随访且资料完整。分析不同夜间睡眠时间(<7、7~9、≥9 h)3年后IGT发生率,采用Logistic回归分析IGT的影响因素,并采用Cox比例风险回归模型评估不同夜间睡眠时间对IGT发生的影响。结果随访时新诊断IGT 396例(14.97%)。夜间睡眠时间<7、7~9、≥9 h的IGT发生率分别为18.25%、13.84%、15.57%(P<0.05)。Logistic回归分析显示,校正多种因素后,与夜间睡眠时间7~9 h比较,<7 h是IGT发生的危险因素(OR 1.378,95%CI 1.063~1.786,P<0.05)。Cox回归分析显示,<7 h的IGT发生风险显著增加(HR 1.303,95%CI 1.032~1.645,P<0.05)。按照年龄、BMI分层后,<7 h的超重/肥胖老年人群IGT发生风险进一步增加(HR 1.579,95%CI 1.032~2.414,P<0.05)。结论夜间睡眠时间不足是中老年人群IGT发生的危险因素,超重/肥胖的老年人群IGT发病风险进一步升高。 Objective To explore the association of nighttime sleep duration and incidence of impaired glucose tolerance(IGT)in middle-aged and elderly residents in communities in Dalian,China.Methods A cohort study was performed by recruiting 6358 middle-aged and elderly residents aged≥40 years old in communities from July to December 2011.2645 participants with normal blood glucose level in base-line were followed-up in 2014.The incidences of IGT by different nighttime sleep durations(<7 h,7~9 h and≥9 h)were calculated.Binary logistic regression models were built to assess the association between nighttime sleep durations(<7,7~9 and≥9 h)and IGT.Cox proportional hazards regression models were built to determine the effect of different nighttime sleep durations on IGT.Results In the 3-year follow-up in 2014,396(14.97%)participants were diagnosed as IGT.The incidences of newly diagnosed IGT in<7,7~9 and≥9 h groups were 18.25%,13.84%and 15.57%,respectively(P<0.05).Logistic regression analysis showed that compared with 7~9 h group,nighttime sleep duration<7 h was an independent risk factor of IGT(OR 1.378,95%CI 1.063~1.786,P<0.05)after adjusting relevant factors.Cox regression analysis demonstrated that<7 h increased the risk of IGT significantly(HR 1.303,95%CI 1.032~1.645,P<0.05).When stratified by age(40~60 years,middle-aged;≥60 years,elderly)and BMI(<24 kg/m;,underweight/normal weight;≥24 kg/m2,overweight/obesity),overweight/obesity elderly individuals with short sleepduration had an increased risk of IGT(HR 1.579,95%CI 1.032~2.414,P<0.05).Conclusion Short nighttime sleep duration was an independent risk factor of IGT in middle-aged and elderly residents,especially in overweight/obesity elderly individuals.
