摘要
为分析和探究弥散性血管内凝血(DIC)患者的血浆D二聚体(D-D)、纤维蛋白原(FIB)检测价值,本文选取2018年9月—2020年11月我院收治的DIC患者35例作为观察组,并将同期35名来自我院体检的健康者作为对照组,对2组研究对象实施血浆D-D、FIB检测,并对检测结果进行对比。结果显示,血浆D-D、FIB指标:观察组DIC患者(3.21±2.88)mg/L、(1.32±0.28)g/L,对照组健康体检者(0.91±0.11)mg/L、(2.81±0.35)g/L,其差异存在显著性,P<0.05;DIC检出例数:血浆D-D检出36例、FIB检出37例,两者联合检测检出35例;诊断准确性、特异性、灵敏度:联合检测指标为100.00%、100.00%、100.00%,血浆D-D检测为67.14%、65.71%、68.57%,FIB检测指标为77.14%、74.29%、80.00%,联合检测数据的指标更高,P<0.05;漏诊率、误诊率:联合检测为0%、0%,血浆D-D检测为31.43%、34.29%、FIB检测为20.00%、25.71%,联合检测的漏诊率和误诊率明显较低,P<0.05。由此可见,在弥散性血管内凝血(DIC)患者中,血浆D-D与FIB联合检测的效果更好,尤其表现在检测的准确性、特异性、灵敏度明显提升,漏诊率、误诊率明显降低,值得临床推广。
In order to analyze and explore the detection value of plasma D-Dimer(D-D)and fibrinogen(FIB)in patients with disseminated intravascular coagulation(DIC),35 patients with DIC treated in our hospital from September 2018 to November 2020 were selected as the observation group,and 35 healthy people from our hospital physical examination in the same period were taken as the control group.The plasma D-D and FIB of the two groups were tested,and the test results were compared.The results showed that the plasma D-D and FIB indexes were(3.21±2.88)mg/L and(1.32±0.28)g/L in the observation group and(0.91±0.11)mg/L and(2.81±0.35)g/L in the control group(P<0.05);Number of DIC cases:36 cases of plasma D-D and 37 cases of FIB,35 cases of combined detection;Diagnostic accuracy,specificity and sensitivity:the combined detection indexes were 100%,100%and 100%,the plasma D-D detection indexes were 67.14%,65.71%and 68.57%,and the FIB detection indexes were 77.14%,74.29%and 80.00%.The indexes of the combined detection data were higher(P<0.05);Missed diagnosis rate and misdiagnosis rate:the combined detection was 0%,0%,plasma D-D detection was 31.43%,34.29%,FIB detection was 20.00%,25.71%.The missed diagnosis rate and misdiagnosis rate of combined detection were significantly lower(P<0.05).It can be seen that in patients with disseminated intravascular coagulation(DIC),the effect of combined detection of plasma D-D and FIB is better,especially in the obvious improvement of detection accuracy,specificity and sensitivity,and the obvious reduction of missed diagnosis rate and misdiagnosis rate,which is worthy of clinical promotion.
