摘要
目的:分析肺透明膜病早产儿通过同步鼻塞式CPAP呼吸机治疗后的临床效果。方法:将2018年1月~2020年1月来本院治疗的60例肺透明膜早产儿为例,分为对照组(n=30)和观察组(n=30),两组均通过肺表面活性物质治疗,然后对照组通过有创通气治疗,观察组通过同步鼻塞式CPAP呼吸机治疗。比较两组患儿的氧疗时间、血气指标、治疗成功率、并发症发生率。结果:观察组患儿的氧疗时间少于对照组(P<0.05),治疗前,两组患儿的PaO_(2)、PaCO_(2)以及治疗前、治疗后1h、12h的pH比较无意义(P>0.05),治疗后观察组患儿的1h、12h的PaO_(2)高于对照组患儿,PaCO_(2)低于对照组患儿(P<0.05);观察组患儿的治疗成功率90.0%高于对照组60.0%(P<0.05);观察组患儿的并发症发生率6.7%低于对照组30.0%(P<0.05)。结论:通过同步鼻塞式CPAP呼吸机治疗肺透明膜早产儿,可以有效缩短氧疗时间,改善患儿的血气分析指标,临床效果优良。
Objective:To analyze the clinical effect of simultaneous nasal obstruction CPAP ventilator in premature patients with pulmonary hyaline membrane disease.Methods:60 premature infants with hyaline membrane treated in our hospital from January 2018 to January 2020 were divided into control group(n=30)and observation group(n=30).Both groups were treated with pulmonary surfactant,then the control group was treated with invasive ventilation,and the observation group was treated with synchronous nasal plug CPAP ventilator.The oxygen therapy time,blood gas index,treatment success rate and complication rate were compared between the two groups.Results:The time of oxygen therapy in the observation group was less than that in the control group(P<0.05).Before treatment,there was no significant comparison between the two groups in PaO_(2),PaCO_(2) and pH before and 1h and 12h after treatment(P>0.05).After treatment,PaO_(2) in the observation group was higher than that in the control group at 1h and 12h,and PaCO_(2) was lower than that in the control group(P<0.05);The treatment success rate of 90.0%in the observation group was higher than 60.0%in the control group(P<0.05);The incidence of complications in the observation group was 6.7%lower than 30.0%in the control group(P<0.05).Conclusion:Synchronous nasal obstruction CPAP ventilator for premature infants with pulmonary hyaline membrane can effectively shorten the oxygen therapy time,improve the blood gas analysis indicators of the children,and the clinical effect is excellent.
