摘要
目的:构建适合我国临床使用的重症监护室(ICU)机械通气病人早期主动活动安全评估量表。方法:以"机械通气危重病人早期主动活动安全标准的专家共识"为概念框架,结合系统性文献回顾以及对16名医护人员访谈结果形成专家咨询问卷;采用德尔菲法,对33名重症医学、重症护理及康复医学专家进行两轮咨询,根据专家意见对量表各项指标进行修改、调整,最终形成ICU机械通气病人早期主动活动安全评估量表。结果:两轮专家咨询的积极系数分别为91.67%、100.00%;权威系数分别为0.900,0.898;第2轮一级、二级、三级指标肯德尔和谐系数依次为0.421,0.346,0.303(P<0.05);最终形成包含心血管系统、呼吸系统、神经系统、其他风险因素及体能评估5个维度的ICU机械通气病人早期主动活动安全评估量表。结论:构建的ICU机械通气病人早期主动活动安全评估量表具有较好的可靠性和权威性,可为临床实施主动活动安全评估提供内容参考和指导。
Objective:To develop the safety assessment scale for early active mobilization in ICU mechanically ventilated patients for clinical use in China. Methods:Based on the "Expert consensus and recommendations on safety criteria for active mobilization of mechanically ventilated critically ill adults" as the conceptual framework,and combined with literature analysis and interviews with sixteen medical and nursing staff,an expert consultation questionnaire was formed. Using Delphi method,thirty-three ICU experts including experienced doctors,nurses and physiotherapists were selected for two rounds of expert consultation. According to the experts′ opinions,the items of the scale were modified and adjusted to form the safety assessment scale for early active mobilization of ICU mechanically ventilated patients.Results:The positive coefficients of the two rounds of expert consultation were 91. 67% and 100. 00%,respectively. The authority coefficients were 0. 900 and 0. 898,respectively. The Kendall′s W of three level items were 0. 421,0. 346,0. 303(P<0. 05). Finally,the safety assessment scale for early active mobilization for ICU mechanical ventilation patients was developed,including five dimensions of cardiovascular system,respiratory system,nervous system,other risk factors and physical fitness assessment. Conclusion:The constructed safety assessment scale for early active mobilization in ICU mechanically ventilated patients had good reliability and authority,which can provide content reference and guidance for the clinical implementation of active activity safety assessment.
