摘要
目的了解河南省三级综合医院外科病房出院准备服务的开展现况,为构建本土化且可操作的出院准备服务模式提供参考。方法采用分层随机抽样的方法,抽取河南省三级综合医院27所,对外科病房的出院准备服务开展现况进行调查。结果共调查了347个外科病房,112个病房开展了出院准备服务,结构层面47.3%的病房组建了出院准备服务实施小组,16.1%有协调者,35.7%有出院转介机制;过程层面有出院准备服务流程的占36.6%;结果层面评价体系不完善。结论仍有待提高出院准备服务开展率。结构层面应明确出院准备服务协调者、完善出院转介机制;过程层面加强多学科协作和患者参与度、建立出院准备服务流程;结果层面应标准化评价指标体系。
Objective:To understand the current situation of discharge preparation service in surgical wards of 3A comprehensive hospitals in Henan Province,and provide reference for the construction of localized and operable discharge preparation service mode.Method:273A comprehensive hospitals in Henan Province were selected to investigate the current situation of discharge preparation services in surgical wards by means of stratified random sampling.Result:A total of 347 surgical wards were investigated,and discharge preparation services were provided in 112 wards.At the structural level,47.3%of the wards had the implementation teams of discharge preparation service,16.1%had coordinators,and 35.7%had discharge referral mechanisms.36.6%had discharge preparation service process;The evaluation system at the result level is not perfect.Conclusion:The rate of discharge preparation service still needs to be improved.At the structure level,the coordinators of discharge preparation service should be defined and the referral mechanism be improved.At the process level,multidisciplinary collaboration and patient participation should be strengthened,and the discharge preparation service process be established.At the result level,the evaluation index system should be standardized.
