摘要
目的:分析前列腺癌与前列腺增生诊断中3.0T MRI的应用价值。方法:2020年1-12月收治前列腺癌患者45例(A组)和前列腺增生45例(B组),两组患者均实施3.0T MRI检查,包括常规MRI检查、弥散加权成像检查(DWI)、动态增强扫描(DCE),以穿刺活检或手术病理检查结果为“金标准”,分析不同患者检查参数指标水平。结果:A组表观扩散系数(ADC)值小于B组,差异有统计学意义(P<0.05);A组峰值、最大线性斜率(SSmax)大于B组,达峰时间小于B组,差异均有统计学意义(P<0.05)。MRI、DWI联合DCE对前列腺癌诊断准确率、特异性、灵敏度均高于单一MRI、DWI、DCE的诊断效能。结论:3.0T MRI在前列腺癌与前列腺增生诊断鉴别方面有重要应用价值,通过多种诊断模式的联合可提高诊断效能,为前列腺癌的早期治疗提供依据。
Objective:To analyze the application value of 3.0T MRI in the diagnosis of prostate cancer and prostatic hyperplasia.Methods:From January to December in 2020,45 cases of prostate cancer(group A)and 45 cases of prostatic hyperplasia(group B)were selected.Both groups of patients underwent 3.0T MRI examination,including conventional MRI examination and diffusion weighted imaging examination(DWI),dynamic enhanced scanning(DCE),using the results of biopsy or surgical pathology as the"gold standard"to analyze the level of examination parameters in different patients.Results:The apparent diffusion coefficient(ADC)value of the group A was smaller than that of the group B,the difference was statistically significant(P<0.05).The peak value and maximum linear slope(SSmax)of the group A were greater than those of the group B,and the peak time was shorter than that of the group B,the differences were statistically significant(P<0.05).The diagnostic accuracy,specificity and sensitivity of MRI,DWI and DCE for prostate cancer were higher than the diagnostic efficiency of single MRI,DWI,and DCE.Conclusion:3.0T MRI has important application value in the diagnosis and differentiation of prostate cancer and prostatic hyperplasia.The combination of multiple diagnostic modes can improve the diagnostic efficiency and provide a basis for the early treatment of prostate cancer.
