摘要
目的:探讨在腰椎间盘退变(LDD)患者髓核组织中半胱氨酸天冬氨酸蛋白酶-1(Caspase-1)、白细胞介素-1β(IL-1β)表达水平及相关性。方法:选取LDD患者40例为观察组,选取同期非退行性椎间盘病变患者43例为对照组。根据Pfirrmann分级系统,观察组又分为轻度病变组(10例)、中度病变组(16例)和重度病变组(14例)。采用实时荧光定量PCR(RT-qPCR)检测Caspase-1、IL-1βmRNA表达水平。采用Western blot法及免疫组化染色检测Caspase-1、IL-1β蛋白表达水平。采用HE染色和病理学评分(HDS)对椎间盘组织的病理损伤程度进行组织学评分。比较各组患者髓核组织Caspase-1、IL-1βmRNA及蛋白表达水平。分析Caspase-1、IL-1βmRNA表达水平与Pfirrmann分级、HDS组织学评分的相关性,以及Caspase-1与IL-1βmRNA表达水平间的相关性。分析LDD患者疾病严重程度的影响因素。结果:观察组Caspase-1、IL-1βmRNA及蛋白表达水平高于对照组(均P<0.05);在观察组中,随着疾病程度逐渐加重,Caspase-1、IL-1βmRNA及蛋白表达水平均逐渐上升(均P<0.05)。免疫组化结果显示,Caspase-1、IL-1β蛋白主要分布于细胞质中,表达水平及趋势同Western blot结果。Spearman相关性分析表明,Caspase-1、IL-1βmRNA表达水平与Pfirrmann分级呈正相关关系(r=0.5386、0.6656,均P<0.05),与HDS评分呈正相关关系(r=0.5585、0.4899,均P<0.05);此外,Caspase-1与IL-1βmRNA表达水平间亦呈正相关关系(r=0.4441,P<0.05)。Logistic回归分析显示,髓核组织中Caspase-1、IL-1βmRNA表达水平与LDD患者疾病严重程度具有相关性,是LDD患者疾病严重程度的影响因素。结论:LDD患者髓核组织Caspase-1、IL-1β表达水平上升,且与疾病严重程度相关。
Objective:To investigate the expression and correlation of Caspase-1 and IL-1βin the nucleus pulposus tissue of patients with lumbar disc degeneration(LDD).Methods:Forty-five patients with LDD were selected as the observation group and 43 patients with non-degenerative disc disease in the same period were selected as the control group.According to Pfirrmann grading system,the observation group was divided into mild lesion group(10 cases),moderate lesion group(16 cases)and severe lesion group(14 cases).Caspase-1 and IL-1βmRNA expression levels were detected by RT-qPCR.Caspase-1 and IL-1βprotein expression levels were detected by Western blot and immunohistochemical staining.HE staining and pathological score(HDS)were used to score the degree of pathological injury of intervertebral disc.Caspase-1 and IL-1βmRNA and protein expression levels in nucleus pulposus were compared between groups.The correlation between Caspase-1 and IL-1βmRNA expression level and Pfirrmann grade,HDS were analyzed.The correlation between Caspase-1 and IL-1βmRNA expression level was analyzed.The influencing factors of disease severity in patients with LDD was analyzed.Results:The expression levels of Caspase-1 and IL-1βmRNA and protein in the observation group were higher than those in the control group(all P<0.05).In the observation group,with the gradual aggravation of the disease,the expression levels of Caspase-1 and IL-1βmRNA and protein increased gradually(all P<0.05).Immunohistochemical results showed that Caspase-1 and IL-1βproteins were mainly distributed in the cytoplasm,and the expression level and trend were the same as those of Western blot.Spearman correlation analysis showed that the expression levels of Caspase-1 and IL-1βmRNA were positively correlated with Pfirrmann grade(r=0.5386,0.6656,all P<0.05)and HDS score(r=0.5585,0.4899,all P<0.05);In addition,there was also a positive correlation between Caspase-1 and IL-1βmRNA expression level(r=0.4441,P<0.05).Logistic regression analysis showed that the expression lev
