摘要
就局部枸橼酸钠抗凝连续性肾脏替代治疗(CRRT)成功救治1例胃癌术后失血性休克致多器官功能障碍患者进行分析。患者诉“上腹部胀痛不适1a余”。诊断:胃恶性肿瘤。患者接受经腹近端胃切除、D2淋巴结清扫术及双通道消化道重建术,术后第6天出现腹腔内大出血、失血性休克、多器官功能障碍、无尿,应用局部枸橼酸钠抗凝行CRRT以改善肾功能。枸橼酸钠抗凝CRRT治疗是抢救肿瘤患者出现出血性休克、无尿、多器官功能障碍安全、有效的手段。
To discuss the successful treatment of a case of multiple organ dysfunction caused by hemorrhagic shock after gastric cancer surgery with local sodium citrate anticoagulant continuous renal replacement thempy(CRRT).The patient complained of“upper abdominal pain and discomfort for more than 1 year”,diagnosed as gastric malignant tumor.The patient accepted transabdominal proximal gastrectomy,D2 lymph node dissection and dual-channel gastrointestinal reconstruction.Massive intra-abdominal hemorrhage,hemorrhagic shock,multiple organ dysfunction and anuria occurred on the 6th day after surgery.CRRT was performed to improve renal function.CRRT treatment is a safe and effective method for patients with hemorrhagic shock,anuri and multiple organ dysfunction.
