摘要
目的探讨急性胃穿孔患者腹腔镜术后发生胃排空障碍综合征的影响因素。方法选取2018年11月至2021年9月商丘市梁园区中医医院接收的158例急性胃穿孔患者作为研究对象,患者均接受腹腔镜下胃大部切除术治疗。记录两组术后胃排空障碍综合征发生情况,收集患者基线资料,分析急性胃穿孔患者腹腔镜术后发生胃排空障碍综合征的影响因素。结果158例患者中,发生胃排空障碍综合征42例,发生率为26.58%;发生组糖尿病、低蛋白血症、幽门梗阻、消化道重建方式与未发生组比较,差异有统计学意义(P<0.05);组间其他基线资料比较,差异无统计学意义(P>0.05);经logistic回归分析结果显示,合并糖尿病、合并低蛋白血症、合并幽门梗阻及毕罗氏Ⅱ式均是急性胃穿孔患者腹腔镜术后胃排空障碍综合征发生的影响因素(OR>1,P<0.05)。结论急性胃穿孔患者腹腔镜术后发生胃排空障碍综合征可能与患者合并糖尿病、低蛋白血症、幽门梗阻及毕罗氏Ⅱ式等因素有关。
Objective To investigate the influencing factors of gastric emptying disorder syndrome after laparoscopic surgery in patients with acute gastric perforation.Methods A total of 158 patients with acute gastric perforation who were treated in Liangyuan District Traditional Chinese Medicine Hospital of Shangqiu City from November 2018 to September 2021 were selected as the research objects,all patients received laparoscopic subtotal gastrectomy in the hospital.The occurrence of postoperative gastric emptying disorder syndrome was recorded of the two groups.The baseline data of patients were collected,and the influencing factors of gastric emptying disorder syndrome after laparoscopic surgery in patients with acute gastric perforation were analyzed.Results Among 158 patients,42 had gastric emptying disorder syndrome,with an incidence of 26.58%.Compared diabetes mellitus,hypoproteinemia,pyloric obstruction and digestive tract reconstruction between occurrence group and non-occurrence group,the difference was statistically significant(P<0.05).There was no statistically significant difference in other baseline data between the two groups(P>0.05).Logistic regression analysis results showed that combined diabetes mellitus,hypoproteinemia,pyloric obstruction and BillrothⅡwere the influencing factors of gastric emptying disorder syndrome after laparoscopic surgery in patients with acute gastric perforation(OR>1,P<0.05).Conclusion Gastric emptying disorder syndrome after laparoscopic surgery in patients with acute gastric perforation may be associated with diabetes mellitus,hypoproteinemia,pyloric obstruction and BillrothⅡ.
