摘要
糖尿病与骨质疏松患病人数众多,已经成为严重的公共卫生问题,两者合并存在对健康危害巨大。糖尿病患者为骨质疏松性骨折高风险人群,可以通过高糖毒性、炎症、氧化应激、胰岛素作用不足等多种机制导致骨代谢紊乱及骨质量下降。应重视糖尿病患者骨折风险评估和防治,通过骨折风险评估工具、双能X线吸收法、骨小梁评分、骨转化标志物等多种方法对糖尿病患者的骨折风险进行综合评估。对于糖尿病患者采取综合管理降低骨折风险,应关注血糖控制对于骨折防治的关键作用,评估糖尿病并发症和降糖药物对于骨代谢的影响,给予个体化的生活方式指导及抗骨质疏松药物治疗,以全面改善患者生活质量。
Diabetes and osteoporosis suffer from a large number of patients and have become a serious public health problem.The combination of the two has great health hazards.Diabetes patients are high-risk groups of osteoporotic fractures,and can cause bone metabolism disorders and bone quality decline through various mechanisms such as high glucose toxicity,inflammation,oxidative stress,and insufficient insulin action.Attention should be paid to fracture risk assessment and prevention of diabetic patients.The fracture risk of diabetic patients can be comprehensively assessed through various methods such as fracture risk assessment tools,dual-energy X-ray absorption,trabecular bone scores,and bone transformation markers.For diabetic patients,comprehensive management is recommended to reduce the risk of fractures.The key role of blood sugar control in fracture prevention and treatment should be paid attention to.Diabetic complications and the impact of hypoglycemic drugs on bone metabolism should be evaluated.Individualized lifestyle guidance and anti-osteoporotic drug treatment should be given in order to comprehensively improve the quality of life of patients.
