Effects of inhalation of nitric oxide combined with high frequency oscillatory ventilation in treatment of neonatal pulmonary hypertension

摘要 目的:观察吸入一氧化氮(NO)联合高频震荡通气(HFOV)治疗新生儿肺动脉高压患儿的效果。方法:选取79例新生儿肺动脉高压患儿作为研究对象,按照随机数字表法分为观察组40例与对照组39例。对照组使用HFOV治疗,观察组在对照组基础上使用NO吸入治疗,比较两组临床疗效以及动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2))、白细胞介素-17(IL-17)、转化生长因子-β_(1)(TGF-β_(1))、血管内皮生长因子(VEGF)和脑钠肽(BNP)水平。结果:观察组治疗总有效率为97.50%,明显高于对照组的79.49%,差异有统计学意义(P<0.05);治疗12、24 h后,观察组PaO_(2)水平高于对照组,PaCO_(2)、IL-17、TGF-β_(1)、VEGF、BNP水平低于对照组,差异有统计学意义(P<0.05)。结论:吸入NO联合HFOV治疗新生儿肺动脉高压患儿可提高治疗总有效率和PaO_(2)水平,降低PaCO_(2)、IL-17、TGF-β_(1)、VEGF和BNP水平,效果优于单纯HFOV治疗。 Objective:To observe effects of inhalation of nitric oxide(NO)combined with high frequency oscillatory ventilation(HFOV)in treatment of neonatal pulmonary hypertension.Methods:79 neonates with pulmonary arterial hypertension were selected as the research objects,and were divided into observation group(40 cases)and control group(39 cases)according to the random number table method.The control group was treated with HFOV,while the observation group was treated with inhalation of NO on the basis of that of the control group.The clinical efficacy and the levels of arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2)),interleukin-17(IL-17),transforming growth factor-β_(1)(TGF-β_(1)),vascular endothelial growth factor(VEGF)and brain natriuretic peptide(BNP)were compared between the two groups.Results:The total effective rate of treatment in the observation group was 97.50%,which was significantly higher than the control group of 79.49%,and the difference was statistically significant(P<0.05).12 and 24 hours after the treatment,the PaO_(2) level in the observation group was higher than that in the control group;the levels of PaCO_(2),IL-17,TGF-β_(1),VEGF and BNP were lower than those in the control group;and the differences were statistically significant(P<0.05).Conclusions:Inhalation of NO combined with HFOV in the treatment of neonatal pulmonary hypertension can improve the total effective rate and the PaO_(2) level,and reduce the levels of PaCO_(2),IL-17,TGF-β_(1),VEGF and BNP.Moreover,it is superior to single HFOV treatment.
