摘要
目的:通过临床研究,探求通督调神针刺法联合西药(叶酸、维生素B12和阿托伐他汀钙片)对高同型半胱氨酸(Hcy)血症患者颈动脉内-中膜厚度(CIMT)及血流动力学的影响,探索更加安全、有效、便捷的缺血性脑卒中早期预警和干预方案。方法:纳入60例符合本研究标准的Hcy患者,随机数字表法分为治疗组(30例)和对照组(30例)。两组患者均接受脑病科常规中西医结合治疗。对照组给予B族维生素(叶酸、维生素B_(12))、阿托伐他汀钙片常规治疗,治疗组在对照组常规治疗基础上联合通督调神针刺干预手法。比较两组患者干预前后血清Hcy、血脂水平和CIMT、狭窄段收缩期峰值流速(PSV)、舒张末期流速(EDV)、阻力指数(RI)变化情况。结果:干预后两组患者血清TC、TG、LDL-C、Hcy以及CIMT、PSV、EDV、RI均较本组干预前明显降低(P<0.05);与对照组比较,治疗组患者干预后血清TC、Hcy水平及CIMT、PSV、EDV、RI降低(P<0.05)。结论:通督调神针刺法联合西药能降低高Hcy血症患者血清Hcy、血脂及CIMT、PSV、EDV、RI,对缺血性卒中高危患者具有早期预警和干预效果。
Objective:To investigate the effect of Tongdu Tiaoshen acupuncture therapy combined with western medicine(folic acid,vitamin B_(12),atorvastatin calcium tablets)on carotid artery intima-media thickness(IMT)and haemodynamics of patients with hyperhomocysteinemia through clinical study,and to explore a safer,effective and convenient early warning and intervention program for ischemic stroke. Methods:A total of 60 patients with hyperhomocysteine meeting the criteria of this study were included and randomly divided into treatment group(30 cases)and control group(30 cases). The two groups received conventional integrated Chinese and western medicine treatment in the department of encephalopathy. The control group received routine treatment with folic acid,vitamin B_(12)and atorvastatin calcium tablets. The treatment group received Tongdu Tiaoshen accupuncture combined with western medicine Changes of serum homocysteine,blood lipid levels and carotid artery IMT、peak systolic velocity(PSV)、end diastolic velocity(EDV)、resistance index(RI)were observed before and after treatment in the two groups. Results:The levels of TC,LDL-C,HDL-C,homocysteine and IMT of carotid artery、PSV、EDV、RI of the two groups were significantly decreased after treatment(P<0.05). Compared with control group,the levels of TC,homocysteine and IMT of carotid artery、PSV、EDV、RI were decreased(P<0.05)in treatment group. Conclusion:Tongdu Tiaoshen acupuncture therapy combined with western medicine can reduce homocysteine,blood lipid levels and carotid artery IMT、PSV、EDV、RI of patients with hyperhomocysteinemia,and has early warning intervention effect for patients with high risk of ischemic stroke.
