摘要
目的探讨子宫内膜癌患者超声血流参数与肿瘤病理分期及预后的相关性。方法选取子宫内膜癌患者100例纳入观察组,另选择同期确诊的子宫内膜增生患者60例纳入对照组。对2组超声基本特征及血流参数搏动指数(PI)、阻力指数(RI)、收缩期峰值流速(PSV)进行比较;对不同病理分期及不同预后子宫内膜癌患者超声血流参数进行比较,利用Spearman分析病理分期与血流参数的相关性,利用受试者工作特征曲线(ROC)分析超声血流参数对患者预后的预测价值。结果观察组病灶形态规则率、内膜低回声率、均匀增强率相比对照组明显升高(P<0.05);观察组PI、RI相比对照组明显降低,PSV相比明显升高(P<0.05)。随着肿瘤分期逐渐升高,患者PI、RI水平明显降低,PSV水平明显上升(P<0.05);Spearman相关性分析结果表明,子宫内膜癌患者病理分期与PI(γ=-0.482,P<0.01)、RI(γ=-0.442,P<0.01)呈负相关,与PSV呈正相关(γ=0.387,P=0.012)。存活组PI、RI相比死亡组明显升高、PSV明显降低(P<0.05);ROC分析结果表明PI、RI、PSV分别预测子宫内膜患者预后的截点值及AUC值分别为(0.48,0.861)、(0.34,0.819)、(45.26 cm/s, 0.862)。结论子宫内膜癌患者超声血流参数与肿瘤病理分期及预后密切相关,PI、RI、PSV对患者病情及预后预测具有良好的价值。
Objective To explore the correlation between ultrasound blood flow parameters and pathological stages of tumors, prognosis in patients with endometrial cancer. Methods A total of 100 endometrial cancer patients treated in the hospital were included in the observation group, while another 60 patients with endometrial hyperplasia confirmed were the control group.The basic characteristics of ultrasound and blood flow parameters [pulsatility index(PI),resistance index(RI),peak systolic velocity(PSV)] were compared between the 2 groups.The ultrasound blood flow parameters among endometrial cancer patients with different pathological stages and prognosis were compared.The correlation between pathological stage and blood flow parameters was analyzed by Spearman.The predictive value of ultrasound blood flow parameters for prognosis was analyzed by receiver operating characteristic(ROC) curves. Results The regular rate of lesions morphology, intimal hypoecho rate and uniform enhancement rate in the observation group were significantly higher than those in the control group(P<0.05),PI and RI were significantly lower than those in the control group, while PSV was significantly higher than that in the control group(P<0.05).With the gradual increase of tumor stages, levels of PI and RI were significantly decreased, while PSV level was significantly increased(P<0.05).The results of Spearman correlation analysis showed that the pathological stages were negatively correlated with PI(γ=-0.482,P<0.01) and RI(γ=-0.442,P<0.01),while positively correlated with PSV(γ=0.387,P=0.012).PI and RI in survival group were significantly higher than those in death group, while PSV was significantly lower than that in death group(P<0.05).The results of ROC curves analysis showed that the cut-off and AUC values of PI,RI and PSV alone for predicting the prognosis of endometrial cancer patients were(0.48,0.861),(0.34,0.819) and(45.26 cm/s, 0.862),respectively.
