摘要
目的探讨贫困县弱势中老年人群的健康相关生命质量现况及其影响因素,为精准健康扶贫及改善慢性病管理水平提供证据。方法基于欧洲五维健康评定量表(EQ-5D)和自设问卷,于2019年7月采用多阶段随机抽样的方法抽取江苏省丰县445名中老年人展开调查。运用秩和检验、CLAD回归模型等进行数据统计分析。结果丰县贫困人群健康相关生命质量EQ-5D效应值中位数为0.812(0.728,1),VAS中位数为79(60,90)。纳入单因素分析中有意义的9个变量进行CLAD回归分析,结果显示,年龄、职业、睡眠时间、高血压、其他慢性病、因慢性病多次门诊和住院会对健康效应值产生一定的影响。结论样本地区慢性病人群生命质量低于非慢性病人群,该人群生命质量的影响因素是多方面的,应根据地方人群特点,制定相应的卫生政策以提高贫困人群的健康相关生命质量。
Objective This paper aims to explore the health-related quality of life and its influencing factors among vulnerable middle-aged and the elderly in poverty-stricken counties,and to provide evidence for accurate health and poverty alleviation and improvement of chronic disease management.Methods Based on EQ-5D and self-designed questionnaire,a multi-stage random sampling method was used to investigate 445 middle-aged and elderly people in Feng county,Jiangsu province in July 2019.The rank-sum test and CLAD model were used to statistical analyse.Results The median EQ-5D value of poor people in Feng county was 0.812(0.728,1),and the median VAS value was 79(60,90).Nine variables included in the univariate analysis were analyzed by CLAD regression.The factors affecting the health utility values among vulnerable people were age,occupation,sleep time,hypertension,other chronic diseases,multiple outpatients,and hospitalization for chronic diseases.Conclusion The quality of life of people with chronic diseases in poor rural areas is lower than that of the general population and the factors affecting the health utility values among vulnerable people in Feng country were various.Therefore,related medical policy should be formulated based on the characteristics of the vulnerable people in order to improving the health related quality of life of the poor.
