摘要
目的探讨子宫内膜癌(EMC)患者全子宫切除术后创伤后成长(PTG)水平,分析影响患者术后PTG的相关因素。方法回顾性分析,收集2014年8月至2018年8月同济大学附属第一妇婴保健院护理部收治的120例接受全子宫切除术治疗的EMC患者资料,根据出院后PTG评定量表评分,将其分为PTG水平正常组(PTG评定量表评分≥70分,75例)和PTG水平低下组(PTG评定量表评分<70分,45例)。比较两组基线资料,采用logistic回归模型分析EMC患者全子宫切除术后PTG的影响因素。结果PTG水平低下组国际妇产科联合会(FIGO)分期为Ⅱ期占比、焦虑自评量表(SAS)评分≥50分占比、抑郁自评量表(SDS)评分≥53分占比及压力知觉量表(PSS)评分高于PTG水平正常组,社会支持量表(SSRS)评分低于PTG水平正常组,差异有统计学意义(P<0.05)。多因素分析结果显示,FIGO分期Ⅱ期、SAS评分≥50分、SDS评分≥53分、高PSS评分是EMC患者全子宫切除术后PTG水平低下的独立危险因素(OR>1,P<0.05),高SSRS评分是其保护因素(OR<1,P<0.05)。结论EMC患者全子宫切除术后PTG水平受社会支持、压力知觉、心理负性情绪、FIGO分期等因素影响。
Objective To investigate the level of post-traumatic growth(PTG)in patients with endometrial carcinoma(EMC)after total hysterectomy,and to analyze the related factors of PTG of patients after operation.Methods The data of 120 EMC patients who received total hysterectomy treatment in the First Maternal and Child Health Hospital Affiliated to Tongji University from August 2014 to August 2018 was collectted,retrospectively.According to the score of PTG inventory after discharge,they were divided into normal level PTG group(score of PTG inventory≥70 points,75 cases)and low PTG level group(score of PTG inventory<70 points,45 cases).The baseline data of the two groups were compared,and logistic regression model was used to analyze the influencing factors of PTG in EMC patients after total hysterectomy.Results The proportions of the International Federation of Gynecology and Obstetrics(FIGO)stage Ⅱ,self-rating anxiety scale(SAS)score≥50,self-rating depression scale(SDS)score≥53,and the scores of per ceived stress scale(PSS)in the low PTG level group were higher than those in the normal level PTG group,and the social support scale(SSRS)score was lower than that of normal PTG group,and the differences were statistically significant(P<0.05).Multivariate analysis showed that FIGO stage Ⅱ,SAS score≥50,SDS score≥53 and high PSS score were independent risk factors for low PTG level after total hysterectomy in EMC patients(OR>1,P<0.05),while high SSRS score was protective factor(OR<1,P<0.05).Conclusion The level of PTG in EMC patients after total hysterectomy is affected by social support,stress perception,negative psychological emotions,FIGO stage,and other factors.
