2型糖尿病肾病患者血清胱抑素C及相关肾功指标的变化研究

Analysis of changes in serum cystatin C and related renal function indexes in patients with type 2 diabetic nephropathy

摘要 目的:探讨血清胱抑素C(CysC)、血尿素(BUN)、血尿酸(UA)以及血肌酐(Cre)联合检测在2型糖尿病肾病诊断中的应用价值.方法:选取从2018年1月~2019年12月在我院住院治疗的2型糖尿病肾病患者153例,按照尿蛋白排泄率(UAER)分3组,单纯糖尿病组(A组)57例,早期糖尿病肾病组(B组)55例,临床糖尿病肾病组(C组)41例.另选取同期来我院健康体检人员53例作为正常对照组.所有研究对象分别检测血清胱抑素C、血尿素、血尿酸、肌酐并进行对比分析.结果:A组与健康对照组各指标比较无明显变化(P>0.05);B组与A组、健康对照组比较,CysC升高明显(P<0.05),BUN、Cre、UA差异无统计学意义(P>0.05);C组与健康对照组、A组、B组比较,各指标差异均有统计学意义(P<0.05);在A组中CysC、BUN、Cre、UA的阳性率为17.5%,10.5%,3.5%,1.8%,在B组中CysC、BUN、Cre、UA阳性率为25.7%,14.5%,10.9%,7.3%,两组中CysC明显高于BUN、Cre、UA的阳性率,差异有统计学意义(P<0.05);联合检测高于单个指标检测的阳性率.结论:CysC、BUN、Cre、UA与糖尿病肾病病程密切相关,CysC能更早的反映糖尿病肾病患者的肾脏损伤情况,联合检测对于糖尿病肾病早期诊断、治疗、病情的监控有重要意义. Objective:To explore the value of combined detection of serum cystatin C(CysC),blood urea(BUN),blood uric acid(UA)and blood creatinine(Cre)in the diagnosis of type 2 diabetic nephropathy.Methods:Select 153 patients with type 2 diabetic nephropathy who were hospitalized in our hospital from January 2018 to December 2019,according to the urine protein excretion rate(UAER),it is divided into 3 groups,there were 57 cases in the simple diabetes group(group A),55 cases in the early diabetic nephropathy group(group B),and 41 cases in the clinical diabetic nephropathy group(group C).In addition,53 patients who came to our hospital for physical examination during the same period were selected as the normal control group.All subjects were tested for serum cystatin C,blood urea,blood uric acid,and creatinine respectively and compared and analyzed.Results:There was no significant change in each index between group A and healthy control group(P>0.05).Compared with group A and healthy control group,CysC increased significantly in group B(P<0.05),there was no statistically significant difference in BUN,Cre,UA(P>0.05).Comparing with the other groups,the group C has statistically differences in all indexs(P<0.05).The positive rates of CysC,BUN,Cre and UA in group A were 17.5%,10.5%,3.5%,1.8%,in group B,the positive rates of CysC,BUN,Cre and UA were 25.7%,14.5%,10.9%,7.3%,CysC in the two groups was significantly higher than the positive rate of BUN,Cre,UA,and the difference was statistically significant(P<0.05).Combined detection is higher than the positive rate of single index detection.Conclusion:CysC,BUN,Cre,UA are closely related to the course of diabetic nephropathy,CysC can reflect the kidney damage of patients with diabetic nephropathy earlier.Combined detection is of great significance for the early diagnosis,treatment,and condition monitoring of diabetic nephropathy.
