摘要
目的探讨层级管理下责任制整体护理模式对输尿管结石手术患者护理质量及围术期身心应激反应的影响。方法选取2017年4月至2019年9月平顶山市第一人民医院行输尿管结石手术患者112例,依据入院时间将患者分为层级管理组(n=56)、常规护理组(n=56)。常规护理组予以常规护理,层级管理组予以层级管理下责任制整体护理模式。比较两组住院时间、住院费用、并发症发生率、护理满意度与术前、术后1 w心理状态〔焦虑自评量表(SAS)、抑郁自评量表(SDS)〕、血清应激反应指标〔去甲肾上腺素(NE)、皮质醇(Cor)〕水平,并统计两组护理人员护理质量评分。结果护理后,两组术后1 w SAS、SDS评分与血清NE、Cor水平较术前降低,且层级管理组较常规护理组低,差异有统计学意义(P<0.05);层级管理组住院时间较常规护理组短,住院费用较常规护理组低,差异有统计学意义(P<0.05);层级管理组并发症发生率为7.14%,低于常规护理组的21.43%,护理满意度为94.64%,高于常规护理组的78.57%,差异有统计学意义(P<0.05);层级管理组护理人员护理质量评分较常规护理组高,差异有统计学意义(P<0.05)。结论层级管理下责任制整体护理模式应用于输尿管结石手术患者可提升护理质量,减轻围术期身心应激反应,减少住院时间、费用与并发症发生情况,提高护理满意度。
Objective To explore the effect of holistic nursing mode of responsibility system under hierarchical management on nursing quality and perioperative physical and mental stress response of patients undergoing ureteral calculus operation.Methods A total of 112 patients undergoing ureteral calculi surgery in the First People's Hospital of Pingdingshan City from April 2017 to September 2019 were selected.According to the admission time,the patients were divided into hierarchical management group and routine care group,with 56 patients in each group.The routine care group was given routine nursing,while the hierarchical management group was given holistic nursing mode of responsibility system under the hierarchical management.The length of hospital stay,the cost of hospitalization,the incidence of complications,the satisfaction degree of nursing and psychological status〔Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS)〕,serum stress response indicators〔norepinephrine(NE),cortisol(Cor)〕levels before and 1 week after surgery of the two groups were compared.The nursing quality score of the two groups was also calculated.Results After nursing,SAS and SDS scores and serum NE and Cor levels of the two groups decreased 1 week after surgery,and the hierarchical management group was lower than that of the routine care group,and the differences were statistically significant(P<0.05).The time of hospitalization in the hierarchical management group was shorter than that in the routine care group,and the hospitalization cost was lower than that in the routine care group,and the differences were statistically significant(P<0.05).The complication rate of the hierarchical management group was 7.14%,lower than 21.43%of the routine care group,and the nursing satisfaction rate was 94.64%,higher than 78.57%of the routine care group,and the differences were statistically significant(P<0.05).The nursing quality score of nursing staff in the hierarchical management group was higher than that in the routine care group,
