摘要
目的:对比我院既往腹腔镜下与开放手术行输尿管再植术治疗输尿管阴道瘘的处理情况及患者结局,进而分析并总结腹腔镜下输尿管再植术的临床效果。方法:回顾性分析了2008年—2018在四川大学华西医院就诊的80例患者资料,依据手术方式分为开放组(60例)和腹腔镜组(20例),比较了两组在住院时间、手术结局及并发症等方面的差异。结果:开放组同腹腔镜组在手术结局(58/60 vs.19/20,P=1),术后并发症(伤口感染:3.33%vs.0,P=1;切口疼痛:11.67%vs.0,P=0.25;慢性尿路感染:5.00%vs.0,P=0.57)等方面差异无统计学意义,住院时间腹腔镜组显著短于开放组(8 d vs.6 d,P<0.001)。结论:对于输尿管阴道瘘这一损伤,腹腔镜下输尿管再植术具有住院时间短、恢复快等优点,且疗效确切,未增加并发症的发生率,值得临床推广。
Objective: To analyze the clinical effects of laparoscopic ureteral replantation by comparing the outcome of patients with ureterovaginal fistula treated with laparoscopic and open ureteral replantation in our hospital. Methods: From 2008 to 2018, 80 patients treated in West China Hospital of Sichuan University were included, and according to the operation method, they were divided into open surgery group(60 cases) and laparoscopic group(20 cases). Collection of the data included hospital stay, operative outcome and postoperative complications. Results: Two groups were same in the surgical outcome(58/60 vs. 19/20, P=1), postoperative complications(wound infection: 3.33% vs. 0, P=1;incision pain: 11.67% vs. 0,P=0.25;chronic urinary tract infection: 5.00% vs. 0, P=0.57), but in hospital stay(8 days vs. 6 days, P<0.001), the laparoscopic group was significantly shorter than the open surgery group. Conclusion: For ureterovaginal fistula, laparoscopic ureteral replantation has the same effect like open surgery with the advantages of short hospital stay, quick recovery, which make it worth clinical promotion.
