摘要
目的探讨围手术康复干预对老年全髋关节置换术(total hip arthroplasty,THA)术后功能水平及运动耐力的影响。方法选取2020年2月—2021年6月火箭军特色医学中心收治的40例老年全髋关节置换术后患者作为研究对象,用随机数字表法分为观察组和对照组,每组各20例。对照组行THA手术前采取常规健康教育,术后4周开始进行康复训练;观察组在健康教育的基础上,增加术前预康复训练及术后早期康复干预。分别于入院时、术前1天、术后第7天、 4周、 8周,采用髋关节功能评分量表(Harris Hip Score,HHS)、视觉模拟评分量表(visual analog scale,VAS)、 6min步行试验(6-minute walk test and daily life, 6MMT)及日常生活活动能力评分(activities of daily living,ADL)对2组的功能情况进行评价。结果入院时,2组HHS评分、VAS评分,6MWT及ADL评分差异无统计学意义(P> 0.05);经过术前预康复后,观察组的HHS评分、VAS评分,6MWT及ADL评分结果优于对照组,差异具有统计学意义(P<0.05)。术后第7天,2组的相关指标差异无统计学意义(P> 0.05);经过8周治疗后,观察组的HHS评分、VAS评分,6MWT及ADL评分结果优于对照组,差异有统计学意义(P<0.05)。结论围手术康复干预有助于提升老年全髋关节置换术后功能水平及运动耐力的提升。
Objective To investigate the effect of perioperative rehabilitation intervention on functional level and exercise endurance of elderly patients after total hip arthroplasty(THA). Methods From February2020 to June2021, 40 elderly patients undergoing total hip arthroplasty treated in Rocket Army Special Medical Center were selected as the research subjects and randomly divided into observation group and control group,with20 cases in each group. Patients in the control group received routine health education before THA operation,and began rehabilitation training4 weeks after the operation.The patients in the observation group received preoperative pre-rehabilitation training and early postoperative rehabilitation intervention based on the routine health education. At admission, 1 day before surgery and7 days, 4 weeks and8 weeks after the operation,Harris Hip Score(HHS),visual analog scale(VAS), 6-minute walk test(6 MMT)and daily life and activities of daily living(ADL)were used to evaluate the functional levels of the patients. Results At admission,there was no significant difference in HHS score,VAS score, 6 MWT and ADL scores between the two groups(P> 0.05). After preoperative pre-rehabilitation,the HHS score,VAS score, 6 MWT and ADL score of the observation group1 day before the operation were better than those of the control group,and the differences were statistically significant(P< 0.05).7 days after the operation,there was no statistically significant difference in the relevant indexes between the two groups(P> 0.05).After8 weeks of treatment,the HHS score,VAS score, 6 MWT and ADL score of the observation group were better than those of the control group(P< 0.05). Conclusion Perioperative rehabilitation intervention is helpful to improve the functional level and exercise endurance of the elderly patients after total hip arthroplasty.
