摘要
目的探讨双能量CT成像技术对痛风的临床诊断价值。方法选取2019年6月至2020年12月山西白求恩医院风湿免疫科收治的83例疑似诊断痛风的患者,均行双能量CT和“金标准”检查,比较两种方法对尿酸盐结晶的检出率;医师对确诊患者行体格检查,并将检查结果与双能量CT结果进行比较。结果双能量CT技术诊断痛风的灵敏度、特异度、阴性预测值、阳性预测值分别为92.31%、83.33%、75%、95.24%;Kappa值为0.73;ROC曲线下面积为0.88(95%CI[0.77~0.99],P<0.01)。较体格检查,双能量CT检出尿酸盐结晶沉积数是前者的4.40倍。双能量CT检查对慢性间歇期痛风患者尿酸盐结晶和骨质破坏的检出率均高于急性期(X^(2)值分别为4.912和8.018,P值分别为0.027和0.005)。结论双能量CT技术可清晰、准确地显示痛风患者尿酸盐沉积情况,可靠性好,具有较高的临床应用价值。
Objective To explore the clinical diagnostic value of dual-energy CT imaging technology for gout.Methods From June 2019 to December 2020,83 patients with suspected gout were selected from the Department of Rheumatology and Immunology of Shanxi Norman Bethune Hospital for dual-energy CT imaging technology examination and“gold standard”examination.The detection rates of urate crystals were compared between the two methods.Clinicians performed physical examination on patients with confirmed gout,and the examination results were compared with the results of urate crystals found by dual-energy CT.Results The sensitivity,specificity,negative predictive value and positive predictive value of dual-energy CT imaging in diagnosing gout were 92.31%,83.33%,75%,and 95.24%,respectively;Kappa value was 0.73;ROC curve area under the curve was 0.88(95%confidence interval[0.77~0.99],P<0.01).Compared with clinical physical examination,the number of urate crystal deposits detected by dual-energy CT imaging was 4.40 times that of the former.The detection rate of urate crystals and bone destruction in patients with chronic intermittent gout by dual-energy CT examination was higher than that in the acute phase(The X^(2) values were 4.912 and 8.018,and the P values were 0.027 and 0.005,respectively).Conclusion The dual-energy CT imaging technology can clearly and accurately display the urate deposition in patients with gout.It has good reliability and high clinical application value.
