摘要
目的分析腹腔镜辅助与开腹直肠癌根治术治疗直肠癌的近期效果。方法60例直肠癌患者,随机分为开腹组和腹腔镜组,各30例。开腹组患者采用开腹直肠癌根治术进行治疗,腹腔镜组采用腹腔镜辅助直肠癌根治术进行治疗。比较两组手术时间、胃肠道蠕动时间、下床行走时间、住院时间、抗生素治疗时间、肠梗阻发生情况、淋巴结清扫数量。结果腹腔镜组手术时间(66.45±3.56)min、胃肠道蠕动时间(17.34±1.81)h、下床行走时间(12.45±2.01)h、住院时间(6.15±1.21)d、抗生素治疗时间(4.25±0.11)d均短于开腹组的(79.34±12.30)min、(24.21±2.34)h、(25.45±3.61)h、(8.24±2.21)d、(6.45±1.50)d,差异有统计学意义(P<0.05)。腹腔镜组肠梗阻发生率为3.33%(1/30),低于开腹组的23.33%(7/30),差异有统计学意义(P<0.05)。腹腔镜组淋巴结清扫数量为(14.45±1.82)枚,开腹组淋巴结清扫数量为(14.25±1.71)枚,两组淋巴结清扫数量比较,差异无统计学意义(P>0.05)。结论直肠癌患者采用腹腔镜辅助直肠癌根治术进行治疗疗效确切,与开腹直肠癌根治术近期效果相近,均可有效清扫淋巴结,但腹腔镜辅助治疗创伤小,并发症少,可加速患者机体的康复进程。
Objective To analyze the recent effect of laparoscopic-assisted resection and open radical resection of rectal cancer in the treatment of rectal cancer.Methods A total of 60 patients with rectal cancer were randomly divided into open group and laparoscopic group,with 30 cases in each group.Patients in the open group were treated with open radical resection of rectal cancer,and the laparoscopic group was treated with laparoscopic-assisted radical resection of rectal cancer.Both groups were compared in terms of operation time,gastrointestinal peristalsis time,off-bed walking time,hospitalization time,antibiotic treatment time,intestinal obstruction occurrence,and the number of lymph node dissections.Results The operation time(66.45±3.56)min,gastrointestinal peristalsis time(17.34±1.81)h,off-bed walking time(12.45±2.01)h,hospitalization time(6.15±1.21)d,antibiotic treatment time(4.25±0.11)d of the laparoscopic group were shorter than(79.34±12.30)min,(24.21±2.34)h,(25.45±3.61)h,(8.24±2.21)d,(6.45±1.50)d of the open group,and the difference was statistically significant(P<0.05).The incidence of intestinal obstruction in the laparoscopic group was 3.33%(1/30),which was lower than 23.33%(7/30)in the open group,and the difference was statistically significant(P<0.05).The number of lymph node dissections in the laparoscopic group was(14.45±1.82)nodes,and the number of lymph node dissections in the open group was(14.25±1.71)nodes,and the difference was not statistically significant(P>0.05).Conclusion Laparoscopic assisted radical resection of rectal cancer is effective in the treatment of rectal cancer patients,and the recent effect is similar to that of open radical resection of rectal cancer,both of which can effectively clear lymph nodes,but laparoscopic adjuvant treatment has less trauma and fewer complications,and can accelerate the recovery of the patients.
