摘要
目的:探讨超声弹性成像应变率比值与甲状腺癌肿瘤细胞生长及血管新生的相关性。方法:选择2017年7月-2019年3月赣州市肿瘤医院收治的41例恶性甲状腺肿块患者作为恶性组,选择2019年4月-2020年12月赣州市肿瘤医院收治的41例良性甲状腺肿块患者作为良性组。所有入组患者均实施超声弹性成像检查,计算应变率比值。比较两组超声弹性成像结节应变率比值,比较两组肿瘤细胞生长指标p53和Ki-67水平、血管内皮生长因子(VEGF)及碱性成纤维细胞生长因子(bFGF),分析应变率比值(SR)与p53、Ki-67、VEGF及bFGF水平变化的相关性。结果:恶性组超声弹性成像结节应变率比值高于良性组(P<0.05);恶性组肿瘤细胞生长指标p53和Ki-67水平均明显高于良性组(P<0.05);恶性组VEGF及bFGF水平均明显高于良性组(P<0.05);SR与p53、Ki-67、VEGF及bFGF水平均呈正相关(P<0.05)。结论:针对甲状腺肿块实施超声弹性成像应变率比值分析,对鉴别肿块良恶性具有一定价值,同时还可反映病灶中细胞生长和新生血管生长情况。
Objective:To investigate the correlation between the strain rate ratio of ultrasound elastography and the growth of thyroid cancer cells and angiogenesis.Method:A total of 41 patients with malignant thyroid masses treated in Ganzhou Cancer Hospital from July 2017 to March 2019 were selected as the malignant group,and 41 patients with benign thyroid masses treated in Ganzhou Cancer Hospital from April 2019 to December 2020 were selected as the benign group.All patients were examined by ultrasonic elastography to calculate the strain rate ratio.The nodule strain rate ratio of ultrasonic elastography was compared between the two groups;the levels of tumor cell growth indexes p53 and Ki-67,vascular endothelial growth factor(VEGF)and basic fibroblast growth factor(bFGF)were compared between the two groups.The correlation between SR and the levels of p53,Ki-67,VEGF and bFGF was analyzed.Result:The strain rate ratio of ultrasound elastography nodules in the malignant group was higher than that in the benign group(P<0.05).The levels of tumor cell growth indexes p53 and Ki-67 in malignant group were significantly higher than those in benign group(P<0.05).The levels of VEGF and bFGF in malignant group were significantly higher than those in benign group(P<0.05).SR was positively correlated with the levels of p53,Ki-67,VEGF and bFGF(P<0.05).Conclusion:The use of ultrasonic elastography strain rate ratio analysis for thyroid masses has a certain value in distinguishing benign and malignant masses,and it can also reflect the growth of cells and new blood vessels in the lesion.
