摘要
目的:探讨危重症新生儿血清降钙素原(PCT)、神经元特异性烯醇化酶(NSE)水平与凝血功能及病情严重程度的关系。方法:选择2019年3月-2020年5月全身炎症反应综合征(SIRS)新生儿167例设为观察组,根据疾病严重程度将其分为轻度组、中度组和重度组;选择同期治疗的非SIRS新生儿74例设为对照组。采用酶联免疫吸附试验测定各组PCT、NSE水平;采用全自动血凝分析仪测定各组凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)及D-二聚体(D-D)水平。对危重症新生儿PCT、NSE与凝血功能及病情严重程度进行Pearson和Spearman相关性分析。结果:观察组危重症新生儿PCT、NSE、APTT及D-D水平均高于对照组(P<0.05);重度组PCT、NSE、APTT及D-D水平均高于轻度组、中度组(P<0.05);Pearson相关性分析结果表明,危重症新生儿PCT、NSE与APTT及D-D水平呈均正相关(P<0.05);Spearman相关性分析表明,危重症新生儿PCT、NSE与患儿病情严重程度呈正相关(P<0.05)。结论:血清PCT、NSE在危重症新生儿中呈高表达,且与凝血功能、病情严重程度存在相关性,加强PCT、NSE水平测定可指导临床治疗,评估患儿疾病严重程度。
Objective:To investigate the relationship between serum procalcitonin(PCT),neuron-specific enolase(NSE)levels and coagulation function and severity of disease in critically ill neonates.Method:A total of 167 neonates with systemic inflammatory response syndrome(SIRS)from March 2019 to May 2020 were selected as the observation group,they were divided into mild group,moderate group and severe group according to the severity of disease;74 neonates without SIRS were selected as the control group.The levels of PCT and NSE were determined by enzyme-linked immunosorbent assay;prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT)and D-dimer(D-D)levels were determined by automatic hemagglutination analyzer.Pearson and Spearman correlation analysis was used to analyze the correlation between PCT,NSE and blood coagulation function and severity of the disease in critically ill neonates.Result:The levels of PCT,NSE,APTT and D-D in critically ill neonates in observation group were higher than those in control group(P<0.05);the levels of PCT,NSE,APTT and D-D in severe group were higher than those in mild and moderate group(P<0.05);Pearson correlation analysis showed that PCT,NSE and APTT and D-D levels were positively correlated(P<0.05);Spearman correlation analysis showed that the PCT and NSE of critically ill neonates were positively correlated with the severity of the disease(P<0.05).Conclusion:Serum PCT and NSE are highly expressed in critically ill neonates,and are correlated with coagulation function and severity of disease.Strengthening the determination of PCT and NSE levels can guide clinical treatment and evaluate the severity of disease in patients.
