摘要
目的分析2016—2020年甘肃省平凉市0-6岁儿童健康变化情况,并提出针对性的防控措施。方法利用甘肃省居民健康档案管理系统,收集整理2016—2020年平凉市0-6岁散居儿童资料,分析营养不良、肥胖、贫血等检出情况。结果 2016—2020年共报告登记新生儿数111 049人,出生时低体重6 230例、发生率5.61%,巨大儿3 123例、发生率2.81%,死亡323例、死亡率2.91‰;新生儿应访视111 049例,访视101 179例、访视率91.11%;3岁以下儿童应系统管理380 148例,实际管理332 351例、管理率87.43%;6岁以下儿童应健康管理754 265例,实际管理679 284例、管理率90.06%;新生儿健康检查480 122例,发现低体重6 223例、发生率1.30%,发现肥胖2 507例、发生率0.52%,贫血16 939例、检出率3.53%,中度贫血1 751例、检出率0.36%;儿童出生时低体重发生率(趋势χ^(2)=134.927)、巨大儿发生率(趋势χ^(2)=113.692)、新生儿访视(趋势χ^(2)=311.811)、3岁以下儿童系统管理(趋势χ^(2)=238.799)和6岁以下儿童健康管理(趋势χ^(2)=439.177)均呈上升趋势(P<0.05),中度贫血检出率(趋势χ^(2)=57.628)呈下降趋势,但死亡率差异无统计学意义(趋势χ^(2)=5.165,P>0.05)。结论平凉市新生儿访视和儿童健康管理工作质量在逐渐完善,但出生时低体重发生率、贫血发生率呈逐年上升趋势;应加强新生儿健康状况监测和儿童保健管理,早期发现异常,预防疾病的发生。
Objective To analyze the status and changes of the health of children aged 0-6 in Pingliang city of Gansu province from 2016 to 2020, and propose targeting measures of prevention and control. Methods The data of scattered children aged 0-6 years in Pingliang city from 2016 to 2020 were collected from Gansu Provincial Resident Health Records Management System for analyzing the detection results of malnutrition, obesity and anemia. Results Totally 111 049 newborns were reported and registered from 2016 to 2020. There were 6 230 newborns of low birth weight, with the incidence rate of 5.61%, 3 123 newborns of giant neonate, with the incidence rate of 2.81%, 323 newborns died, with the mortality rate of 2.91%. All 111 049 newborns should be visited, and 101 179 newborns were visited with the visiting rate of91.11%. There were 380 148 children under 3 year-old systematically managed, and 332 351 children actually under management with the management rate of 87.43%, and 754 265 children under 6 years old should be systematically managed,and 679 284 children actually under management with the management rate of 90.06%. Among the 480 122 infants received health examination, 6 223 infants suffered from low weight, with the incidence rate of 1.30%, 2 507 infants were obesity with the incidence rate of 0.52%, 16 939 infants had anemia with the detection rate of 3.53%, and 1 751 infants were moderate anemia with the detection rate of 0.36%. Rising trend(P<0.05) showed in the incidence rates of low birth weight(trend χ^(2)=134.927), incidence rate of macrosomia(trend χ^(2)=113.692), the visits rates of the newborns(trend χ^(2)=311.811), systematic management of children under 3 year-old(trend χ^(2)=238.799), and health management of children under 6 years old(trend χ^(2)=439.177). But there was downward trend in the detection rates of moderate anemia(trend χ^(2)=57.628, P<0.05). There was no statistical difference in the death rates(trend χ^(2)=5.165, P>0.05).
