摘要
目的探讨血流感染(BSI)患者降钙素原(PCT)、C反应蛋白(CRP)、白细胞(WBC)计数、中性粒细胞比率(NEU)水平变化及临床意义。方法回顾性分析2019年6月至2020年6月本院收治的100例BSI患者的临床资料,所有患者经血培养实验最终确诊病毒感染42例作为病毒组,细菌感染58例作为细菌组。检测受检者PCT、CRP、WBC、NEU水平,分析细菌感染病原菌分布情况及细菌感染科室分布情况,并分析病毒组与细菌组PCT、CRP、WBC、NEU变化水平及与BSI的相关性。结果 100例BSI患者经血培养实验显示,细菌组共分离出病原菌58株,包括革兰阳性菌14株,革兰阴性菌33株,真菌11株;细菌感染率为58.00%,其中重症加强护理病房(ICU)检出率最高,为76.19%,外科检出率最低,为0.00%。细菌组PCT、CRP、WBC、NEU水平均高于病毒组,差异有统计学意义(P<0.05)。以PCT、CRP、WBC、NEU水平变化作为自变量,将BSI感染类型作为因变量,进行线性回归检验,结果显示,PCT、CRP、WBC、NEU水平变化与BSI患者病情呈正相关(P<0.05),得到线性回归方程:Y=–4.100+0.002X1+0.079X2+1.117X3+0.009X4。结论 PCT、CRP、WBC、NEU水平与BSI患者病情呈正相关,可作为BSI早期诊断及临床治疗的重要指标。
Objective To investigate the changes and clinical significance of procalcitonin(PCT), C-reactive protein(CRP), white blood cell(WBC) count and neutrophil ratio(NEU) levels in patients with bloodstream infection(BSI). Methods The clinical data of 100 patients with BSI admitted to our hospital from June 2019 to June 2020 were retrospectively analyzed. All patients were finally diagnosed with viral infection through blood culture experiments as the virus group, and 58 patients with bacterial infection as the bacterial group. The level of PCT, CRP, WBC, NEU of the subjects were detected, analyzed the distribution of bacterial infection pathogens and the distribution of bacterial infection departments, and analyzed the correlation between the change levels of PCT, CRP, WBC, NEU and BSI between the virus group and the bacterial group. Results The blood culture experiment of 100 BSI patients showed that 58 pathogens were isolated from the bacterial group, including 14 Gram-positive bacteria,33 Gram-negative bacteria, and 11 fungi. The bacterial infection rate was 58.00%. The highest detection rate in intensive care unit(ICU) was76.19%, and the lowest detection rate in surgery was 0.00%. The levels of PCT, CRP, WBC and NEU in the bacterial group were higher than those in the virus group, and the difference was statistically significant(P<0.05). The changes in PCT, CRP, WBC, and NEU levels were used as independent variables, and BSI infection types were used as dependent variables. The linear regression test results showed that the changes in PCT, CRP, WBC,and NEU levels were positively correlated with the condition of BSI patients(P<0.05). The linear regression equation is obtained as Y=–4.100+0.002 X1+0.079 X2+1.117 X3+0.009 X4. Conclusion The levels of PCT, CRP, WBC, and NEU are positively correlated with the condition of BSI patients, and can be used as important indicators for early diagnosis and clinical treatment of BSI.
