摘要
目的 探讨内镜下胆道支架置入术(EBS)和经皮肝穿刺胆道支架置入术(PTBS)对不可切除胰腺癌所致黄疸的疗效。方法 回顾性研究2013年6月至2017年11月42例不可切除胰腺癌所致黄疸的患者资料,其中22例成功接受PTBS,20例成功接受EBS。比较两组患者胆道引流的有效性、并发症和生存情况。结果 两组总胆红素在术后第3、7、14天均显著下降,但两组间胆汁引流的有效性比较,差异无统计学意义(P>0.05)。PTBS组平均生存时间为(10.64±2.90)个月,与EBS组平均生存时间(10.75±2.53)个月比较,差异无统计学意义(P>0.05)。两组生存曲线比较,差异无统计学意义(P>0.05)。两组早期并发症及晚期并发症比较,差异无统计学意义(P>0.05)。结论 PTBS和EBS均可以作为不可切除胰腺癌所致黄疸的姑息治疗,胆道引流效果确切,并发症少。
Objective To explore the efficacy of endoscopic biliary stenting(EBS)and percutaneous transhepatic biliary stenting(PTBS)on jaundice caused by unresectable pancreatic cancer.Methods A total of 42 patients with jaundice caused by unresectable pancreatic cancer from June 2013 to November 2017 were retrospectively studied,of which 22 patients successfully received PTBS and 20 patients successfully received EBS.The effectiveness,complications and survival of biliary drainage in the patients of two groups were compared.Results The levels of total bilirubin were decreased significantly in both groups on the 3rd,7th,and 14th days after surgery,but there was no significant difference in the effectiveness of bile drainage between the two groups(P>0.05).The mean survival time in the PTBS group was(10.64±2.90)months,and there was no significantly difference than that of(10.75±2.53)months in the EBS group(P>0.05).There was no significant difference in terms of survival curves between the two groups(P>0.05).There were no significant differences in terms of early complications and late complications between the two groups(P>0.05).Conclusion Both PTBS and EBS can be used as palliative treatment for jaundice caused by unresectable pancreatic cancer,with definite effect of biliary drainage and fewer complications.
