骨凿、CAS-kit和超声骨刀行羊上颌窦内提升效果对比的实验研究

摘要 目的通过建立的上颌窦内提升山羊模型,比较骨凿法、CAS-kit液压法及超声骨刀法经牙槽嵴顶上颌窦提升时提升高度、穿孔率及操作时间的差异,观察并分析3种方法提升窦膜的效果。方法收集30个新鲜羊头(60侧上颌窦),随机分为3组,每组20个上颌窦,采用CBCT图像确定提升位点,分别用骨凿、CAS-kit及超声骨刀将上颌窦提升至9 mm,直至窦膜穿孔停止实验。记录窦膜最终提升高度、窦膜穿孔率和操作时间。结果超声骨刀组和CAS-kit组的提升高度大于骨凿组,差异有统计学意义(P<0.001);CAS-kit组和超声骨刀组之间无明显差异(P=0.385)。骨凿组、CAS-kit组、超声骨刀组穿孔率分别为90.0%,40.0%,15.0%,三组间比较差异有统计学意义(P<0.001)。骨凿组的操作时间短于超声骨刀组及CAS-kit组(P<0.001);CAS-kit组和超声骨刀组之间操作时间的比较亦有统计学差异(P=0.032)。结论从提升高度上来看,骨凿组的提升高度有限,低于超声骨刀组和CAS-kit组;从穿孔率来说,骨凿组提升至9 mm时穿孔率较高,而CAS-kit组次之,超声骨刀组穿孔率最低;从操作时间上讲,骨凿组的操作时间最短,远低于超声骨刀组和CAS-kit组。 Objective To evaluate the sinus lifting efficacy of osteotome,CAS-kit,and piezosurgery by comparing safe elevation,perforation rate,and operation time in an goat model of crestal sinus elevation.Methods Thirty fresh goat heads(60 sides of maxillary sinuses)were collected and randomly divided into three groups with 20 maxillary sinuses in each group.The lifting sites were defined according to CBCT images.The maxillary sinuses were lifted to 9 mm by osteotome,CAS-kit and piezosurgery,respectively,and the experiment was stopped until the sinus membrane was perforated.Finally,the final elevation,the sinus perforation rate and the operation time were recorded.Results The final elevation in piezosurgery group and CAS-kit group was significantly higher than that in osteotome group(P<0.001).There was no significant difference in final elevation between CAS-kit group and piezosurgery group(P=0.385).The perforation rate was 90.0%in osteotome group,40.0%in CAS-kit group,and 15.0%in piezosurgery group,and there was significant difference among three groups(P<0.001).The operation time in osteotome group was shorter than that in piezosurgery group and CAS-kit group(P<0.001),and there was significant difference between CAS-kit group and piezosurgery group(P=0.032).Conclusion The lifting height of maxillary sinus by osteotome is limited,which is lower than that of piezosurgery and CAS-kit.The perforation rate is the highest in osteotome group when it is raised to 9 mm,and it is the lowest in piezosurgery group.The operation time is the shortest in osteotome group,lower than that in piezosurgery group and CAS-kit group.
