摘要
目的探讨鼠神经生长因子在外伤性视神经挫伤中的应用效果。方法选取2018年4月至2021年1月安阳市眼科医院收治的65例外伤性视神经挫伤患者作为研究对象,并按照随机数表法将其随机分为研究组(33例)与对照组(32例),对照组患者采用常规药物治疗,研究组患者在对照组治疗的基础上联合应用鼠神经生长因子治疗,对比两组患者治疗前后最佳矫正视力、视觉诱发电位(VEP)、平均光敏感度(MS)与视野平均缺损(MD)变化情况,以及血、尿常规及血生化指标异常情况。结果治疗后,研究组患者最佳矫正视力明显优于对照组(Z=-2.048,P=0.041);VEP⁃P100波振幅、MS明显高于对照组(t=2.915、4.320,P=0.005、P<0.001),VEP⁃P100波潜伏期、MD明显低于对照组(t=2.866、6.271,P=0.006、P<0.001);研究组患者血白细胞计数(WBC)、血红细胞计数(RBC)、淋巴细胞计数(LY)、中性粒细胞计数(NEUT)、血红蛋白(Hb)、血小板计数(PLT)、红细胞比容(HCT)、尿WBC、尿RBC、尿糖(GLU)、尿蛋白(PRO)、尿酮体(KET)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、肌酐(Cr)、血尿素氮(BUN)、总胆红素(TBil)异常情况与对照组无明显差异(χ^(2)=0.318、0.000、0.000、2.001、0.985、0.383、0.001、0.383、0.318、0.985、1.047、0.985、0.383、1.047、0.128、0.318、0.985,P=0.573、0.982、0.982、0.157、0.321、0.536、0.975、0.536、0.573、0.321、0.306、0.321、0.536、0.306、0.721、0.573、0.321)。结论鼠神经生长因子治疗外伤性视神经挫伤可明显提高视网膜神经节细胞的生理功能,促进视功能恢复,疗效显著,且安全性较高。
Objective To study the effect of applying mouse nerve growth factor in treating traumatic optic nerve contusions.Methods 65 patients with traumatic optic nerve contusions admitted to Anyang Eye Hospital between April 2018 and January 2021 were enrolled and divided,using random number table,into study group(n=33)and control group(n=32).Patients in control group were treated with conventional drugs,while patients in study group were jointly treated with mouse nerve growth factor besides the conventional drugs used in control group.The following indexes such as pre⁃and post⁃treatment best corrected visual acuity,changes of visual evoked potential(VEP),mean light sensitivity(MS)and mean visual field defect(MD),and abnormalities in blood,urine and blood biochemical examinations were compared between the two groups.Results After treatment,the best corrected visual acuity in study group was significantly superior to that in control group(Z=-2.048,P=0.041),the VEP⁃P100 wave amplitude and MS were significantly higher than control group(t=2.915 and 4.320,P=0.005,P<0.001),and VEP⁃P100 wave latency and MD were significantly lower than control group(t=2.866 and 6.271,P=0.006,P<0.001).The abnormalities of WBC,RBC,LY,NEUT,Hb,PLT and HCT in blood,WBC,RBC,GLU,PRO and KET in urine,and ALT,AST,Cr,BUN and TBil in study group didn’t differ significantly from control group(χ^(2)=0.318,0.000,0.000,2.001,0.985,0.383,0.001,0.383,0.318,0.985,1.047,0.985,0.383,1.047,0.128,0.318 and 0.985,P=0.573,0.982,0.982,0.157,0.321,0.536,0.975,0.536,0.573,0.321,0.306,0.321,0.536,0.306,0.721,0.573 and 0.321).Conclusion Mouse nerve growth fac⁃tor in the treatment of traumatic optic nerve contusions can significantly improve the physiological function of retinal ganglion cells and facilitate the restoration of visual function,and the therapeutic benefits were definite,with high safety.
