摘要
目的探讨不同肠内营养泵入时间对胃癌术后患者营养状况、肠内营养不耐受情况及睡眠情况的影响。方法选取2020年2月-2021年2月在某三级甲等医院胃肠外科行胃癌全切术后给予鼻肠管肠内营养的患者共90例,采用随机数字表法将其分成3组,每组30例,A组分3次泵入,每次持续泵入6 h,间断2 h;B组采用持续16 h泵入法;C组采用持续24 h泵入法。比较3组术前1 d与术后第8天体质量、血红蛋白、白蛋白、总蛋白及患者睡眠质量评分,以及肠内营养期间不耐受的发生情况。结果3组体质量、血红蛋白、白蛋白及总蛋白在组间效应比较,差异无统计学意义(P>0.05)。3组肠内营养期间恶心呕吐、腹胀、腹泻及便秘发生情况比较,差异也无统计学意义(P>0.05);睡眠情况比较,差异有统计学意义(F_(组间)=9.531,P<0.001)。结论3种泵入方法对患者的营养状况及不耐受情况的发生无影响,但16 h泵入组可以改善患者的睡眠质量。
Objective To explore the effect of different enteral nutrition pumping on the nutritional status,enteral nutrition intolerance and sleep of patients with gastric cancer after operation.Methods Ninety patients receiving nasointestinal tube enteral nutrition after the total resection of gastric cancer in a Grade-3 Class-A hospital between February 2020 and February 2021 were selected,and divided into group A,B and C,each of 30,according to a random number table.Group A was given 6-hour pumpings with an interval of 2 hours,3 times a day.Group B was pumped 16 hours with a break of 8 hours,while group C was pumped 24 hours continuously.The body weight,hemoglobin,albumin and total protein,as well as the sleeping quality of the patients before the operation and on the 8th day after the operation were recorded and compared.The occurrence of intolerance during enteral nutrition was also observed.Results There was no significant difference in the body weight,hemoglobin,albumin and total protein among the three groups(P>0.05).No significant differences were observed in terms of nausea,vomiting,abdominal distention,diarrhea and constipation among the 3 groups(P>0.05).However,There were statistically significant difference the sleeping quality of 3 groups(F_(group)=9.531,P<0.001).Conclusion Compared with 24-hour pumping or 6-hour pumping with 2-hour break,16-hour pumping has no effect on nutrition status and intolerance status,but can improve the sleep quality of patients.
