支撑喉镜内镜下低温等离子射频消融术治疗会厌囊肿的疗效观察 被引量:2

Observation on the Curative Effect of Low-temperature Plasma Radiofrequency Ablation Under Support Laryngoscope Endoscopy in the Treatment of Epiglottic Cysts

摘要 目的探讨支撑喉镜内镜下低温等离子射频消融术治疗会厌囊肿患者的疗效。方法选取长垣市人民医院2020年1~12月收治的会厌囊肿者86例,其中接受传统切除术治疗的43例患者作为对照组,另1:1比例选取同期接受支撑喉镜内镜下低温等离子射频消融术治疗的43例患者作为研究组,统计对比两组围术期指标(术中出血量、黏膜修复时间、手术时间、术后疼痛时间)、随访6个月复发率和术前、术后7 d会厌囊肿评分、术前、术后3 d应激反应[皮质醇(Cor)、去甲肾上腺素(NE)]。结果研究组手术、黏膜修复、术后疼痛时间少于对照组,术中出血量低于对照组(P<0.05);术后7 d两组会厌囊肿分值低于术前,且研究组低于对照组(P<0.05);随访6个月,研究组复发率2.33%低于对照组18.60%(P<0.05);术后3 d两组Cor、NE高于术前,但研究组低于对照组(P<0.05)。结论支撑喉镜内镜下低温等离子射频消融术治疗会厌囊肿者效果明显,可优化手术指标,缩短术后疼痛与黏膜修复时间,改善临床症状,减轻创伤应激,抑制复发。 Objective To investigate the efficacy of low-temperature plasma radiofrequency ablation under support laryngoscope endoscopy in the treatment of epiglottic cysts.Methods A total of 86 patients with epiglottic cysts in Changyuan City People’s Hospital from January 2020 to December 2020 were selected.Among them,43 patients who underwent traditional resection were selected as the control group.The other 1:1 ratio was selected to receive support laryngoscope during the same period.43 patients treated with low-temperature plasma radiofrequency ablation under the microscope were used as the research group.The two groups were statistically compared with perioperative indicators(intraoperative blood loss,mucosal repair time,operation time,postoperative pain time),recurrence rate and 6-month follow-up Preoperative and 7 days postoperative epiglottic cyst score,preoperative and 3 days postoperative stress response[cortisol(Cor),norepinephrine(NE)].Results The time of surgery,mucosal repair,and postoperative pain in the study group was less than that of the control group,and the amount of intraoperative blood loss was lower than that of the control group(P<0.05);the epiglottic cyst scores of the two groups were lower than that of the preoperative 7 days after operation,and the study group was lower In the control group(P<0.05);follow-up for 6 months,the recurrence rate of the study group was 2.33%lower than that of the control group 18.60%(P<0.05);Cor and NE in the two groups were higher than before the operation on the 3 rd day after operation,but the study group was lower In the control group(P<0.05).Conclusion Low-temperature plasma radiofrequency ablation under support laryngoscope endoscopy is effective in treating epiglottic cysts.It can optimize surgical indicators,shorten postoperative pain and mucosal repair time,improve clinical symptoms,reduce traumatic stress,and inhibit recurrence.
