摘要
目的:分析男性患者精子DNA碎片率和年龄、精液常规参数(精子数、浓度、前向动力、总活力、形态)及临床因素(糖化血红蛋白,乙肝表面抗原,精浆弹性蛋白酶)之间的相关性。方法:随机收集2020年12月到2021年3月男性精液标本142例。按照精子DNA碎片率(DFI)将患者分为组1:0%~15%,组2:15%~30%,及组3:>30%组。采用基于吖啶橙染色法的流式细胞仪检测精子DNA碎片率,按照世界卫生组织(WHO)标准检测精液各项参数。采用Graph prism7.0及R语言软件进行统计分析及图片绘制,分析精子DFI和年龄、精液常规参数及临床因素之间的相关性。结果:精子总数、浓度、前向运动力、总活力和正常形态比例随着DFI的升高而显著降低(P<0.05)。组2的糖化血红蛋白水平显著高于组1(P<0.05)。DFI与年龄、糖化血红蛋白水平、HDS呈正相关,并与精液体积、总精子数量、浓度、前向运动力、精子总活力及正常形态比例呈负相关(P<0.05)。结论:DNA碎片率可以很好反映精子质量。因此精子DNA碎片检测可作为一项重要的精子质量评估指标,为全面、客观评估男性生育力提供了有效信息。
Objective:To evaluate the correlation among sperm DNA fragmentation index(DFI),age,conventional semen parameters(including sperm volume,counts,motility,and morphology),and clinical parameters[including glycosylated hemoglobin(HbA1c),hepatitis B surface antigen,seminal and plasma elastase]in males.Methods:Randomly included were semen samples from 142 male subjects in our hospital between December 2020 and March 2021.The patients were divided into three groups based on DFI values:group 1(DFI<15%),group2(DFI 15%~30%),and group3(DFI>30%).The DFI was measured by flow cytometry with acridine orange(AO)staining.In addition,the semen parameters were determined according to the World Health Organization(WHO)standards.Graph Prism 7.0 and R software were used for statistical analysis and figure generation.Thereby,the correlation among sperm DFI,age,conventional semen parameters,and clinical parameters were analyzed.Results:The sperm count,concentration,forward motility,total motility,and the proportion of normal morphology decreased significantly with higher DFI levels(P<0.05).The HbA1c level in group 2 was significantly higher than that in group 1(P<0.05).DFI was positively correlated with age,HbA1c level,and high DNA stainability(HDS),ans was negatively correlated with total sperm counts,volume,concentration,forward motility,total motility,and proportion of sperms with normal morphology.Conclusion:Sperm DNA fragmentation index could well reflect the quality of sperm,and therefore can be important as a quality control indicator to provide effective information for comprehensive and objective evaluation of male fertility.
