盆底肌功能锻炼对结直肠肿瘤术后肛门括约肌的影响 被引量:2

摘要 目的观察盆底肌功能锻炼对结直肠肿瘤腹腔镜辅助自然腔道取标本手术(laparoscopic-assisted natural orifice specimen extraction,LA-NOSE)后肛门括约肌的效果。方法选择本院2018年4月—2019年9月接收的60例中结直肠肿瘤LA-NOSE手术后患者,根据随机数字表法分为对照组和试验组,各30例。对照组术后护理常规,试验组基于术后护理常规,采用盆底肌肉锻练。术后1、3、6个月采用Wexner评分评价患者的肛门排便功能;比较两组患者术前术后肛门持续收缩时间。结果(1)两组患者术后1、3、6个月Wexner评分呈降低的趋势,与术后1个月对比,术后3、6个月Wexner评分明显降低(P<0.05);与术后3个月对比,术后6个月Wexner评分明显降低(P<0.05)。与对照组对比,试验组术后1、3、6个月的Wexner评分明显更低(P<0.05)。(2)与术前对比,两组术后持续收缩时间明显延长(P<0.05),与对照组对比,试验组患者术后持续收缩时间明显更长(P<0.05)。结论盆底肌功能锻炼对结直肠肿瘤LA-NOSE手术后肛门括约肌的效果确切,肛门有节律的运动能够刺激肠壁神经末梢,促使肠道蠕动加快,进而促使粪便从肠道排出,可以有效地改善患者的排便功能,有利于促进患者手术后康复,值得广泛应用。 Objective To observe the effect of functional exercise of pelvic floor on anal sphincter after laparoscopic-assisted natural orifice specimen extraction(LA-NOSE)operation of colorectal tumor.Methods A total of 60 patients with colorectal tumor after LA-NOSE operation received by our hospital from April 2018 to September 2019 were divided into control group and experimental group according to the random number table method,30 cases in each group.The control group was given postoperative routine nursing,and the experimental group was given pelvic floor muscle exercise based on postoperative routine nursing.One month,three months and six months after the operation,Wexner score was used to evaluate the anal defecation function of the patients,and the continuous anal contraction time before and after the operation was compared between the two groups.Results(1)The Wexner scores of the two groups decreased at 1 month,3 months and 6 months after operation.Compared with 1 month after operation,the Wexner scores decreased significantly at 3 months and 6 months after operation(P<0.05);compared with 3 months after operation,the Wexner scores decreased significantly at 6 months after operation(P<0.05).Compared with the control group,the scores of Wexner in the experimental group were significantly lower at 1,3 and 6 months after operation(P<0.05).(2)Compared with the preoperative,the postoperative continuous contraction time of the two groups was significantly longer(P<0.05).Compared with the control group,the postoperative continuous contraction time of the experimental group was significantly longer(P<0.05).Conclusion The effect of pelvic floor exercise on anal sphincter after LA-NOSE operation is accurate.The rhythmic movement of anus can stimulate the nerve endings of intestinal wall,accelerate the peristalsis of intestinal tract,and then promote the excretion of feces from intestinal tract.It can effectively improve the defecation function of patients and promote the recovery of patients after operation.It is worth wide
