摘要
目的观察子宫内膜癌患者外周血中性粒细胞与淋巴细胞比值(NLR)、D-二聚体(D-D)表达与预后的关系,评估其预测价值。方法回顾性收集2016年5月至2020年1月于我院接受腹腔镜治疗且预后未复发的50例患者(未复发组),收集同时期内于我院接受腹腔镜治疗但术后复发的50例患者(复发组),查阅病历资料,记录并比较两组入院时外周血中NLR、D-D表达情况,分析上述指标与子宫内膜癌患者腹腔镜术后复发情况的关系。结果与未复发组子宫内膜癌患者相比,复发组患者NLR、D-D均呈高表达(P<0.05),组间其他基线资料比较差异无统计学意义(P>0.05);经Logistic回归分析结果显示,子宫内膜癌患者NLR、D-D过表达可能是导致患者腹腔镜术后复发的风险因子(P<0.05);绘制受试者ROC曲线分析,得到曲线下面积(AUC)NLR0.869,D-D0.922,诊断价值较高;当NLR与D-D联合检测时,AUC最高,为0.952;且当外周血NLR、D-D分别为1.931、0.657 ng/ml时,可获得最佳预测价值。结论子宫内膜癌患者入院时外周血中NLR、D-D表达情况与腹腔镜术后复发关系密切,临床可通过检测患者外周血中NLR、D-D表达情况,针对NLR、D-D高表达患者,给予针对性的干预措施,旨在减少子宫内膜癌患者腹腔镜术后复发风险。
Objective To observe the relationship between the peripheral blood neutrophil-to-lymphocyte ratio(NLR)and D-dimer(D-D)expression and the prognosis in patients with endometrial cancer and to assess their predictive value.Methods Fifty patients with endometrial cancer received laparoscopic treatment in our hospital from May 2016 to January 2020 were retrospectively collected.The patients had not relapsed(non-recurrence group).Another 50 patients with endometrial cancer and recurrence in our hospital during the same period were also collected(recurrence group).Medical records were reviewed.NLR and D-D expression in peripheral blood at the time of admission were recorded and compared between the two groups.The relationship between the indexes and the recurrence after laparoscopic surgery was analyzed.Results Comparedwith the non-recurrence group,NLR and D-D were higher in the recurrence group(P<0.05).However,there was no statistically significant difference in other baseline data between the two groups(P>0.05).Logistic regression analysis showed that higher NLR and over-expressed D-D in patients with endometrial cancer were risk factors for recurrence afterlaparoscopic surgery(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of NLR was 0.869 and D-D was 0.922,with high diagnostic value.When NLR and D-D were combined,the AUC was the highest(0.952).The best predictive value was obtained when the peripheral blood NLR and D-D were 1.931 and 0.657 ng/ml,respectively.Conclusion NLR and D-D in peripheral blood of endometrial cancer patients at admission are closely related to post-laparoscopic recurrence.Clinically,targeted intervention measures can be given to patients with high NLR and D-D to reduce the risk of recurrence in patients with endometrial cancer after laparoscopic surgery.
