摘要
【目的】探讨牙周牙髓联合病变患者根管治疗后不同时机进行牙周治疗的临床疗效。【方法】选取空军军医大学第三附属医院急诊与综合临床科及西安大兴医院口腔科2018年10月至2019年10月诊治的牙周牙髓联合病变患者120例为研究对象,以抽签随机法分为观察组与对照组,每组60例。两组均在根管治疗后进行牙周治疗,观察组在根管治疗1周后进行牙周治疗,对照组在根管治疗3周后进行牙周治疗。比较两组患者治疗后5周的临床疗效、治疗前后牙周探诊深度(PD)、附着丧失水平(AL)、牙龈退缩(RG)、菌斑指数(PLI)及龈沟液和血清白介素-8(IL-8)、IL-6、IL-1β水平。【结果】观察组患者总有效率91.66%(55/60),与对照组的88.33%(53/60)比较差异无统计学意义(P>0.05);两组患者治疗后PD、AL、PLI均明显下降,RG明显上升,与治疗前比较差异均有统计学意义(P<0.05),观察组患者治疗后PD、AL、PLI水平明显低于对照组,其差异有统计学意义(P<0.05);两组患者治疗后龈沟液和血清IL-8、IL-1β、IL-6水平均明显下降,与治疗前比较差异有统计学意义(P<0.05),观察组患者治疗后的IL-8、IL-1β、IL-6水平明显低于对照组,其差异有统计学意义(P<0.05)。【结论】根管治疗联合早期牙周治疗用于牙周牙髓联合病变可更好改善患牙牙周指数、抑制炎症反应,值得临床推广应用。
【Objective】To explore the curative effect of periodontal treatment at different time after root canal therapy in patients with combined periodontal and pulp lesions.【Methods】A total of 120 patients with combined periodontal and pulp lesions from October 2018 to October 2019 were selected as the research object.Patients were randomly divided into the observation group and the control group with 60 cases in each group.Both groups received periodontal treatment after root canal treatment.Patients in the observation group received periodontal treatment in 1 week after root canal treatment and patients in the control group received periodontal treatment in 3 weeks after the root canal treatment.The clinical efficacy of the two groups were compared 5 weeks after treatment,and the differences of periodontal probing depth(PD),attachment loss level(AL),gingival retreat(RG)and plaque index(PLI)before and after treatment were compared between the two groups as well.In addition,interleukin-8(IL-8),interleukin-6(IL-6)and interleukin-1β(IL-1β)levels in gingival crevicular fluid and serum were compared between the two groups before and after operation.【Results】The total effective rate of observation group was 91.66%(55/60),and there was no statistical significance compared with 88.33%(53/60)of control group(P>0.05).The levels of PD,AL and PLI in the two groups were significantly decreased and the level of RG was significantly increased after treatment,and the differences were statistically significant(P<0.05).The levels of PD,AL and PLI in the observation group were significantly lower than those in the control group after treatment,and the differences were statistically significant(P<0.05).Compared to before operation,levels of IL-8,IL-1βand IL-6 in gingival crevicular fluid and serum in both groups decreased significantly after operation with statistical significance(P<0.05),while levels of IL-8,IL-1βand IL-6 in observation group were lower than those in control group after operation and the difference was sta
