摘要
目的探讨阴道锥体训练联合生物反馈盆底肌治疗老年女性压力性尿失禁(SUI)效果。方法选取2017年1月~2018年12月哈尔滨医科大学附属第一医院收治的老年女性SUI患者84例,根据随机数字表法分为对照组与试验组,分别实施常规盆底肌训练与阴道锥体训练联合生物反馈盆底肌治疗。观察对比两组临床总有效率、盆底肌力恢复情况、漏尿频率评分、最大尿道闭合压与腹压漏尿点压。结果试验组疗效优于对照组,差异有统计学意义(P<0.05)。治疗后两组盆底肌力评分比较,差异有统计学意义(P<0.05),试验组盆底肌力评分级别高的患者较多,恢复的较好。治疗后,试验组漏尿频率评分低于对照组,最大尿道闭合压与腹压漏尿点压高于对照组,差异均有统计学意义(P<0.05)。结论针对老年女性SUI患者采取阴道锥体训练联合生物反馈盆底肌治疗效果显著,对改善患者尿动力学参数,促进患者盆底肌力恢复,减少漏尿频率均具有明显的作用,建议临床推广应用。
Objective To investigate the effect of vaginal cone training combined with biofeedback pelvic floor muscle therapy on stress urinary incontinence(SUI) in older women.Methods A total of 84 older women with SUI treated in the First Affiliated Hospital of Harbin Medical University from January 2017 to December 2018 were selected.They were randomly divided into the control group and experimental group.The control group received conventional pelvic floor muscle training, and the experimental group received vaginal cone training combined with biofeedback pelvic floor muscle therapy.The overall response rate, pelvic floor muscle strength, urinary leakage score, maximum urethral closure pressure and abdominal urine leak point pressure were assessed in the two groups.Results The therapeutic effect in the experimental group was better than that in the control group, and the difference was statistically significant(P<0.05).After treatment, there was a significant difference in the pelvic floor muscle strength score between the two groups(P<0.05),and there were more patients with high pelvic floor muscle strength scores in the experimental group.After treatment, the score of urine leakage frequency was lower, and the scores of the maximum urethral closure pressure and abdominal urine leak point pressure were higher in the experimental group than those in the control group(P<0.05).Conclusion Vaginal cone training combined with biofeedback pelvic floor muscle therapy may achieve satisfactory efficacy for older patients with SUI,which has significant effects on improving patients’ urodynamic parameters, enhancing patients’ pelvic floor muscle strength, and reducing the frequency of urine leakage.
