摘要 目的探讨合并腹型肥胖的2型糖尿病(T2DM)患者内脏脂肪面积(VFA)的影响因素。方法回顾性分析2018年6月~2020年3月北京大学第一医院内分泌科收治的T2DM患者728例,采用生物电阻抗法测定VFA,分为T2DM合并肥胖组(VFA≥100 cm^(2))395例、单纯T2DM组(VFA<100 cm^(2))333例。比较两组临床资料差异,Pearson分析VFA与各指标的相关性,多元线性回归分析VFA的影响因素。结果T2DM合并肥胖组体质指数(BMI)、颈围、腰围、腰臀比(WHR)、收缩压(SBP)、舒张压(DBP)、皮下脂肪面积(SFA)、VFA、三酰甘油(TG)、总胆固醇(TC)、尿酸(UA)、胰岛素抵抗指数(HOMA-IR)、三碘甲状腺原氨酸(T_(3))、游离T_(3)(FT_(3))均高于单纯T2DM组,游离T_(4)(FT_(4))低于单纯T2DM组(P<0.05)。Pearson法分析显示,VFA与BMI、颈围、腰围、WHR、SBP、DBP、SFA、TG、UA、HOMA-IR、T_(3)、FT_(3)呈正相关,与FT_(4)呈负相关,与FBG、HbA1c、TC无线性相关。多元线性回归分析结果显示,BMI、腰围、TG、FT_(3)可正向影响VFA,FT_(4)可负向影响VFA。结论T2DM合并腹型肥胖患者VFA随BMI、腰围、TG、FT_(3)增加而增多,随FT_(4)增高而减少。 Objective To explore the influential factors of visceral fat area(VFA)in type 2 diabetes mellitus(T2DM)patients with abdominal obesity.Methods A retrospective study was conducted to analyze the clinical data of 728 patients with T2DM who were hospitalized in the First Hospital of Peking University from June 2018 to March 2020.The patients were divided into T2DM with obesity group(395 cases with VFA≥100 cm^(2))and pure T2DM group(333 cases with VFA<100 cm^(2))according to VFA measured by bioelectrical impedance analysis.The differences in clinical data between the two groups were compared.Pearson analysis was used to evaluate the correlation between VFA and various indicators,and multiple linear regression was used to analyze the influential factors of VFA.Results Body mass index(BMI),neck circumference,waist circumference,waist-to-hip ratio(WHR),systolic blood pressure(SBP),diastolic blood pressure(DBP),subcutaneous fat area(SFA),VFA,tri acylglycerol(TG),total cholesterol(TC),uric acid(UA),insulin resistance index(HOMA-IR),triiodothyronine(T_(3)),and free T_(3)(FT_(3))in T2DM with obesity group were higher than those in pure T_(2)DM group,free T_(4)(FT_(4))was lower than that in pure T2DM group(P<0.05).Pearson analysis showed that VFA was positively correlated with BMI,neck circumference,waist circumference,WHR,SBP,DBP,SFA,TG,UA,HOMA-IR,T_(3) and FT_(3);negatively correlated with FT_(4);and had no linear correlation with FBG,HbAlc and TC.Multiple linear regression analysis showed that BMI,waist circumference,TG,FT_(3) positively affected VFA,and FT_(4) negatively affected VFA.Conclusion VFA of T2DM patients with abdominal obesity may increase with the increase of BMI,waist circumference,TG,FT_(3),and decrease with the increase of FT_(4).
