摘要
目的探讨团体性心理治疗对老年结直肠癌患者心理痛苦的干预效果。方法方便选取昆明医科大学第一附属医院2017年9月—2019年3月肿瘤科结直肠癌患者128例,随机分配干预组(n=64)和对照组(n=64)。干预组除常规护理外,接受每3周1次,共3次的团体心理治疗,对照组仅给予临床常规护理。使用心理痛苦温度计(distress thermometer,DT)比较两组干预前后心理痛苦程度的差异。结果干预前,干预组和对照组DT得分差异无统计学意义(P>0.05);干预后,干预组和对照组无时间精力照顾老人/孩子占比分别为4.76%和28.13%、周围环境占比19.05%和43.75%、紧张占比4.76%和28.13%、担忧占比26.98%和54.69%、对日常活动丧失兴趣占比17.46%和42.19%、睡眠占比53.97%和76.56%,差异有统计学意义(χ^(2)=10.221、8.979、5.694、10.077、9.258、7.158,P<0.05)。结论团体性心理治疗对老年结直肠癌患者心理痛苦的干预有较好效果。
Objective To investigate the effect of group psychotherapy on the psychological distress of elderly patients with colorectal cancer.Methods A total of 128 colorectal cancer patients in the Oncology Department of the First Affiliated Hospital of Kunming Medical University from September 2017 to March 2019 were conveniently selected and randomly assigned to the intervention group(n=64)and the control group(n=64).In addition to routine nursing care,the intervention group received group psychotherapy once every 3 weeks for a total of 3 times,while the control group was given clinical routine nursing only.The distress thermometer(DT)was used to compare the differences in the degree of psychological distress between the two groups before and after intervention.Results Before intervention,there was no statistically significant difference in DT score between the intervention group and the control group(P>0.05).After the intervention,the proportion of no time and energy to take care of the elderly/children in the intervention group and control group was 4.76%and 28.13%,the surrounding environment accounted for 19.05%and 43.75%,stress accounted for 4.76%and 28.13%,worry accounted for 26.98%and 54.69%,loss of interest in daily activities accounted for 17.46%and 42.19%,sleep accounted for 53.97%and 76.56%,the difference was statistically significant(χ^(2)=10.221,8.979,5.694,10.077,9.258,7.158,P<0.05).Conclusion Group psychotherapy has a good effect on the psychological distress of elderly patients with colorectal cancer.
