摘要
目的:本研究采用芒针速刺法治疗子宫内膜异位患者,从肠道菌群角度分析其治疗机理,以探讨芒针法在子宫内膜异位中的治疗效果。方法:取40名子宫内膜异位患者为研究对象,随机分为两组,对照组给予米非司酮给药治疗,而芒针组则在药物治疗基础上给予芒针配合治疗,治疗时间为3个月经周期。在治疗完成1个月经周期后,对患者进行随访,对其疼痛评分(NRS)、血清中的脂多糖、前列腺素E2、雌二醇和炎性因子(IL-1、TNF-α及IL-10)、粪便微生物的多样性(香浓指数、辛普森指数、Chao1及ACE)和门水平进行分析。结果:在给予患者芒针速刺治疗后,患者的NRS评分显著低于对照组(P<0.05),血清中的脂多糖和前列腺素E2也显著低于对照组(P<0.05),雌二醇差异无统计学意义(P>0.05),而血清中炎性因子也均显著低于对照组(P<0.05),粪便中的α-多样性指标均显著高于对照组(P<0.05),厚壁菌门、拟杆菌门、放线菌门和蓝藻菌门的比例显著高于对照组(P<0.05),而变形菌门的比例则显著低于对照组(P<0.05)。结论:芒针速刺可以通过改善患者肠道微生物多样性和组成结构、抑制脂多糖向患者血清的分泌、降低血清中疼痛刺激因素和炎性因子的水平来提高子宫内膜异位患者的治疗效果。
Objective:To study the mechanism and the effect of elongated fast needlingin the treatment of endometriosis(EM)based on intestinal flora.Methods:40 patients with EM were randomly divided into the control group and the study group.The control group was treated with Mifepristone;on which basis,the study group was also treated with elongated fast needling.The treatment time was 3 menstrual cycles.The patients were followed up for pain score(NRS),serum lipopolysaccharide,prostaglandin E2,estradiol and inflammatory factors(IL-1,TNF-α,IL-10),fecal microbial diversity(Shannon index,Simpson index,Chao1,ACE)and gate level were analyzed during the first cyle period after the treatment.Results:After elongated fast needling,the NRS score decreased significantly compared to that in the control group(P<0.05),the serum levels of lipopolysaccharide and prostaglandin E2 were also significantly decreased compared to those in the control group(P<0.05),but there was no significant difference in estradiol(P>0.05).Compoored to those in the control group,serum inflammatory factors were also significantly lower(P<0.05),the diversity indexes in feces were significantly higher(P<0.05),and the proportion of Firmicutes,Bacteroides,Actinobacteria and Cyanobacteria in feces increased significantly(P<0.05),whereas the proportion of Proteobacteria decreased significantly in the study group after the treatment(P<0.05).Conclusion:Elongated fast needling can inhibit the secretion of lipopolysaccharide in the serum of patients by improving the diversity and composition of intestinal microorganisms,and reduce the levels of pain stimulating factors and inflammatory factors in the serum,so as to improve the therapeutic effect in the treatment of EM.
