肺磨玻璃结节影像组学参数测量的可重复性研究 被引量:1

摘要 目的:研究肺磨玻璃结节(GGO)影像组学参数测量的可重复性及参数与GGO良、恶性诊断的关系。方法:选取医院收治的76例GGO患者(共76个<3 cm的GGO)的临床、病理及CT影像学资料,按病理结果将其分为良性GGO组(37例)和恶性GGO组(39例)。由2名经验丰富的放射科医师分别采用自动化全局和结节中心画取两种方案进行影像图像处理及参数测试,间隔12个月后由其中1名主治医生重新测试一次。采用组内相关系数(ICC)对2名医师的测量(ICC-1)一致性和同一医师的2次测量(ICC-2)一致性进行分析。结果:采用自动化全局测量方案进行测量时有11个参数的测量结果一致性均很好,ICC-1范围为0.809~0.997,ICC-2范围为0.803~0.996。采用结节中心画取测量方案进行测量时有4个参数的测量结果一致性均很好,ICC-1范围为0.819~0.997,ICC-2范围为0.949~0.959。两种方案灰度共生矩阵参数中,相关性参数的测量一致性均较低,ICC为0.720~0.842。两组GGO的短径、平均径、圆度、角二阶矩及熵比较差异有统计学意义(t=3.169,t=3.944,t=3.312,t=2.660,t=3.865;P<0.05)。结论:自动化测量获取的影像组学参数的可重复性较高,而且能为良、恶性GGO的诊断提供有效信息。 Objective:To explore the repeatability of the parameters measurement of radiomics on lung ground glass nodule(GGO),and the relationship between the parameters and the diagnoses of benign and malignant GGO.Methods:The clinical,pathological and imaging data of 76 patients with GGO(total of 76 GGOs within 3 cm)admitted to hospital were retrospectively analyzed.They were divided into benign GGO group(37 cases)and malignant GGO group(39 cases)according to pathological results.Two practiced radiologists respectively adopted automatic whole situation plan and drawing from center of nodule to process pictures and test parameters.After 12 months,one of two attending doctors retest them again.The interclass correlation coefficient(ICC)was adopted to analyze the measured consistency between two doctors(ICC-1)and the measured consistency of the same doctor between two measurements(ICC-2).Results:The consistencies of the measured results of 11 parameters were favorable in the measurement with automatic whole situation plan,and ICC-1 range was 0.809-0.997,and ICC-2 range was 0.803-0.996.The consistencies of the measured results of 4 parameters were favorable in the measurement by drawing from center of nodule,and ICC-1 range was 0.819-0.997,and ICC-2 range was 0.949-0.959.In the parameters of co-occurrence matrix of grey of two plans,the measured consistencies of ICC were lower,and ICC was 0.720-0.842.The differences of short diameter,average diameter,circular degree,the secondary moment of angle and entropy of GGO between two groups were significant(t=3.169,t=3.944,t=3.312,t=2.660,t=3.865,P<0.05).Conclusion:The repeatability of automatic measurement is higher in obtaining radiomics parameters,and it can provide effective information for the diagnoses of benign and malignant GGO.
