超声引导下糖皮质激素注射治疗单侧慢性跖筋膜炎的效果研究 被引量:1

摘要 目的分析单侧慢性跖筋膜炎患者临床治疗中采取超声引导下糖皮质激素注射方案的有效性。方法56例单侧慢性跖筋膜炎患者,使用随机数字表法分为研究组及对照组,每组28例。两组均注射倍他米松和利多卡因,对照组采取压痛点注射,研究组采取超声引导下注射。比较疗效及干预前后疼痛程度、足底筋膜厚度。结果研究组治疗总有效率92.86%高于对照组的67.86%,差异具有统计学意义(P<0.05)。干预后,研究组视觉模拟评分法(VAS)评分为(2.09±0.78)分,低于对照组的(4.55±0.45)分,差异具有统计学意义(t=14.455,P=0.000<0.05)。干预后,研究组足底筋膜厚度为(0.20±0.10)cm,小于对照组的(0.57±0.15)cm,差异具有统计学意义(t=10.860,P=0.000<0.05)。结论对于单侧慢性跖筋膜炎而言,临床治疗中采取超声引导下注射倍他米松联合盐酸利多卡因可获得较为理想的效果,是一种值得推荐的治疗模式。 Objective To analyze the effectiveness of glucocorticoid injection in the treatment of unilateral chronic plantar fasciitis under ultrasound guidance.Methods A total of 56 patients with unilateral chronic plantar fasciitis were divided into research group and control group according to random numerical table,with 28 cases in each group.Both groups were injected with betamethasone and lidocaine,with pressure point injections in the control group and ultrasound-guided injections in the research group.The efficacy,pain degree and plantar fascia thickness before and after the intervention were compared between the two groups.Results The total effective rate of treatment 92.86%in the research group was higher than 67.86%of the control group,and the difference was statistically significant(P<0.05).After the intervention,the visual analogue scale(VAS)score was(2.09±0.78)points in the research group,which was lower than(4.55±0.45)points in the control group,and the difference was statistically significant(t=14.455,P=0.000<0.05).After the intervention,the plantar fascia thickness in the research group was(0.20±0.10)cm,which was smaller than(0.57±0.15)cm in the control group,and the difference was statistically significant(t=10.860,P=0.000<0.05).Conclusion In the clinical treatment of unilateral chronic plantar fasciitis,the ultrasound-guided injection of betamethasone combined with lidocaine can obtain relatively ideal results,which is a recommended treatment mode.
