摘要
目的分析产前彩超和磁共振检查在前置胎盘并植入诊断中的应用价值。方法选择本院2012年6月~2020年5月收治的前置胎盘并疑似胎盘植入患者60例作为本次研究对象,所有患者均先后予以产前彩超与磁共振检查,以分娩术后病理结果作为金标准,对比产前彩超、磁共振和两者联合检查前置胎盘并植入的诊断敏感性、特异度、准确性。结果产前彩超和磁共振联合诊断胎盘植入的准确率为96.67%,特异度为100.00%,敏感度为96.36%。产前彩超和磁共振联合诊断的准确率、敏感度均高于单独产前彩超、磁共振检查,差异有统计学意义(P<0.05);三种检查方法特异度及单独产前彩超、磁共振检查准确率、特异度、灵敏度差异无统计学意义(P>0.05)。结论产前彩超、磁共振均可用于前置胎盘并植入的检查,但效果有限,将两种检查方法联合诊断可大大提高诊断的准确率,便于临床制定分娩方案,以此降低母婴的风险,改善母婴的预后。
Objective To analyze the application value of prenatal color ultrasound and magnetic resonance imaging in the diagnosis of placenta previa and and placenta accreta.Methods 60 patients with placenta previa combined with suspected placenta accreta admitted in our hospital from June 2012 to May 2020 were chosen as the research objects.All patients were treated successively with magnetic resonance imaging and prenatal color ultrasound.Postoperative pathologic results were chosen as the gold standard to compare diagnostic sensitivity,accuracy and specificity among prenatal color ultrasound,magnetic resonance(NMR)and the combined examination.Results The accuracy,specificity and sensitivity of ultrasonography and magnetic resonance imaging in the diagnosis of placental accreta were 96.67%,100.00%and 96.36%respectively.The accuracy and sensitivity of combined examination were higher than those of single prenatal color ultrasound or single MRI,and the difference was statistically significant(P<0.05).There were no statistically significant differences in specificity or sensitivity among the three methods(P>0.05).Conclusion Both prenatal color ultrasound and magnetic resonance imaging can be used for the examination of placenta previa and accreta,but the effect is limited.Combined diagnosis of two methods can greatly improve the accuracy of diagnosis and facilitate to make delivery plan,so as to reduce the maternal and infant risk and improve maternal and infant outcomes.
