摘要
目的:探讨开启双腔起搏器的心室自身优先(VIP)功能对接受双腔起搏器植入术的病态窦房结综合征患者术后左心功能的影响。方法:选取2018年1月至2019年12月期间在昆明市延安医院接受双腔起搏器植入术的100例病态窦房结综合征患者为研究对象。按照术后是否开启双腔起搏器的VIP功能将其均分为观察组和对照组。术后,为对照组患者不开启双腔起搏器的VIP功能,仅使用其常规功能。术后1周,为观察组患者开启双腔起搏器的VIP功能。术后12个月,对比两组患者临床症状的改善情况、起搏器相关并发症的发生情况、起搏器相关的参数及心功能相关的指标。结果:术后12个月,对两组患者进行随访的结果显示,其晕厥等症状均消失,均未出现与起搏器相关的并发症。两组患者心房、心室的起搏阈值、心房的起搏比例、左房内径(LAD)及左室射血分数(LVEF)相比,P>0.05。与对照组患者相比,观察组患者心室的起搏比例较低,其左室舒张末内径(LVEDD)较短,其进行6分钟步行试验(6MWT)时步行的距离较长,P<0.05。结论:为接受双腔起搏器植入术的病态窦房结综合征患者术后开启起搏器的VIP功能可明显降低其心室的起搏比例,改善其LVEDD及6MWT的指标,有利于保护其心功能。
Objective:To investigate the effect of ventricular self-priority(VIP)function of dual-chamber pacemaker on left heart function in patients with pathological sinus syndrome after dual-chamber pacemaker implantation.Methods:A total of 100 patients with pathological SINUS syndrome who received dual-chamber pacemaker implantation in Yan‘an Hospital of Kunming city from January 2018 to December 2019 were selected as the study subjects.All patients were divided into observation group and control group according to whether the VIP function of double-chamber pacemaker was turned on after operation.After surgery,the VIP function of the dual-chamber pacemaker was not turned on for the control group,and its routine function was only used.One week after surgery,the VIP function of dual-chamber pacemaker was turned on for the observation group.Twelve months after surgery,the improvement of clinical symptoms,the incidence of pacemaker relat ed complications,pacemaker related parameters and cardiac function related indicators were compared between the two groups.Results:Twelve months after surgery,the results of follow-up showed that the symptoms of syncope and other symptoms disappeared in both groups,and no pacemaker related complications occurred.The pacing threshold of atrium and ventricle,pacing ratio of atrium,left atrial diameter(LAD)and left ventricular ejection fraction(LVEF)in 2 groups were compared,P>0.05.Compared with the control group,patients in the observation group had a lower ventricular pacing ratio,a shorter left ventricular end diastolic diameter(LVEDD),and a longer walking distance during the 6-minute walking test(6MWT),P<0.05.Conclusion:For patients with pathological SINUS syndrome who received double-chamber pacemaker implantation,the VIP function of the pacemaker after operation can significantly reduce the pacing ratio of their ventricles,improve their LVEDD and 6MWT indexes,and help to protect their cardiac function.
